Collaborates with the RM Director in developing and presenting risk management orientation materials for all new Kaweah Health staff and new managers. Collaborates with the RM Director and clinical department mangers for on-going staff education related to risk management and risk prevention. Coordinates DHS visits and reporting activities: Maintains current DHS filing system, appropriately labeled, easily retrievable, and ensures purging and storage of files per RM Department policy. Under the direction of the RM Director, coordinates and assists Kaweah Health Directors' development of action plans, and/or written responses. Ensures appropriate follow-up actions and monitoring activities are completed, and all documentation is included in the DHS file. Provides assistance to the VP's and PI Director on any DHS survey visits. Collaborates with the PI Department staff to improve processes. Meets weekly with the PI Director, PI staff, RM Director to review high severity occurrences. Identifies trends in the RM data from the Occurrence Reporting database which require process improvements, and communicates to the PI staff and appropriate Kaweah Health Director as applicable. Assists the RM Director in Kaweah Health wide risk management and prevention activities: On direction of the RM Director, establishes a regular monthly schedule to visit all Kaweah Health campuses and units/departments to evaluate their clinical risk issues and risk prevention activities. Provides a written monthly report of those visits to the RM Director that identifies areas visited, what was evaluated, results of the evaluation with special note of any areas of concern for risk, and any recommendations for improvements. Additional Attends and actively participates in appropriate Kaweah Health committees as directed by the RM Director providing timely and appropriate feedback to the Director. Manages the RM Department and Kaweah Health RM activities in the absence of the RM Director. Responsible for responding to any RM issues referred to the department through the Occurrence Reporting database: Monitors the database at least daily for any Occurrences transmitted to the RM queue. Performs investigations in a timely manner and documents results in the database. Notifies the RM Director immediately of any high severity (as defined by the JCAHO Sentinel Event and DHS Adverse Event policy, AP. 87) or high-risk occurrences. Provides the RM Director a monthly report of RM referred occurrences with a brief analysis including any identified trends and accompanying risk reduction strategy recommendations if necessary. Makes recommendations to the RM Director for any revisions to the database to improve data collection and reporting. Position: Cardiac Advanced Practice Provider Monogram Health is seeking a Cardiac Advanced Practice Provider for the delivery of personalized, compassionate medical care to patients primarily with heart failure and other cardiac conditions. The Advanced Practice Provider will be responsible for caring for patients, maintaining accurate and current patient records and scheduling, administering follow-up appointments as required, and supporting the training and clinical readiness of local market APPs caring for patients with cardiac conditions in the home. The successful candidate will work as a team with our physician specialists, field-based nurses, community health workers, and physicians to deliver premium care to every patient. Primary duties include patient health assessment, creating strategies to improve or manage a patient’s cardiac condition, introducing habits for health promotion, and helping local market clinicians apply evidence-based cardiac care practices in the in-home setting. He/she may also conduct physical exams, order tests, prescribe medications, and serve as a coordinator with the patient's primary healthcare provider and their specialists. This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state. Compensation The base salary ranges for this role are listed below by state and reflect the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors. Develop and reinforce practical training on cardiac condition recognition, escalation criteria, medication management, patient education, care coordination, and documentation standards for local market APPs delivering home-based care. Partner with cardiology leadership and local market clinical teams to identify education needs, close knowledge gaps, and promote consistent application of evidence-based cardiac protocols across markets. Work closely with cardiology team, other specialist teams, and clinical care teams. Conduct assessments on patients both in the patients' home and in the virtual environment. Counsel and educate patients and families about benefits and programs available to help them live healthier lives. Document items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan. Complete all documentation and paperwork in a timely manner. Maintain quality of care standards as defined by the practice. Prescribe medications, order tests, and collaborate with patient’s Monogram cardiologist and physician.
