The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care. The Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices. Responsibilities: Participates in the coordination and planning of the delivery of quality, cost-effective hospice care. Evaluates patient services provided by the interdisciplinary team. Maintains working knowledge of hospice state licensure, federal and accreditation requirements. Monitor services and documentation to meet hospice policies and procedures, state, federal, and accreditation guidelines. Serve as a liaison to the Administrator regarding clinical challenges and progress in patient care.
