Essential Duties and Responsibilities Provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and oral care. Assist residents with meals, feeding, hydration, and maintaining proper nutrition. Safely transfer, reposition, and ambulate residents according to their care plans. Answer call lights promptly and respond appropriately to resident needs. Obtain and record vital signs and other assigned measurements. Observe residents for changes in condition, behavior, skin integrity, appetite, mobility, or overall well-being and promptly report concerns to the nurse. Maintain a clean, safe, and comfortable resident environment. Assist with incontinence care and maintain appropriate hygiene and infection-control practices. Provide respectful and compassionate care while protecting resident privacy and dignity. Assist residents with personal belongings and encourage independence whenever possible. Participate in resident activities and promote positive social interaction. Complete required documentation accurately and timely. Follow all facility policies, procedures, safety standards, and infection-prevention practices. Work cooperatively with nurses, STNAs, residents, families, and other members of the healthcare team. Perform other duties as assigned by nursing leadership. Qualifications Must hold a current and valid Ohio STNA certification . Previous experience in a skilled nursing or long-term care setting is preferred but not required. Must demonstrate good communication and interpersonal skills. Must be dependable, responsible, and able to work effectively as part of a team. Must be able to provide compassionate care to residents with varying physical and cognitive needs. Must be able to follow written and verbal instructions. Physical Requirements Ability to stand and walk for extended periods. Ability to assist with lifting, transferring, repositioning, and ambulating residents. Ability to push or pull equipment and supplies as required. Ability to bend, stoop, kneel, and reach as needed to perform resident-care duties. Ability to safely perform the essential functions of the position with or without reasonable accommodation. JOB DESCRIPTION SUMMARY Assists the clinical department and program office with the clerical duties associated with all aspects of medical record keeping, chart audits, DME management, aide scheduling and office support. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Maintains and updates patient admission files in PAS. 2. Completes admission audit and communicates issues/problems to the Clinical Director. 3. Inputs updates into current software system any information or changes relating to insurance, level of care, staff assignment and/or discharge. 4. Scans and files all paper clinical data, hand signed orders and consents into patient records. 5. Ensures plan of care/comprehensive assessment update/recertification are opened in a timely manner to allow staff to document their summaries prior to team. 6. Ensures plan of care/comprehensive assessment update/recertification are resolved once approved by Clinical Director or designee. 7. Tracks and emails as appropriate certification/recertification statements/POC/CA and all orders. 8. Runs unresolved orders report weekly to track unsigned orders. 9. Maintains patient on-call book. 10. Contacts physician office, hospital or nursing facility in a timely manner in order to obtain current information, signatures or any other information pertinent to the patient’s admission and their hospice chart. 11. Assists as needed with providing information necessary for annual report and other patient data which may be requested. 12. Receives visit logs from all staff. 13. Inputs all visit logs of staff not electronically documenting into current software system and compares visit logs to contact visit reports weekly. 14. Tracks and ordering durable medical equipment (DME) and medical supplies. 15. If requested, participates in team meeting and is responsible for ensuring all documentation and electronic records are present as requested at team meeting. 16. Responsible to maintain the team meeting sign-in sheet, and ensures that the team agenda is attached. 17. Responsible for closing clinical/bereavement charts, and preparing charts beyond thirteen (13) months so that they may be moved to off-site storage. 18. Communicates with the Clinical Director in order that the charts are sent to off-site storage. 19. Maintains and orders office supplies. 20. Schedules and coordinates the hospice aide daily assignments to ensure the patient receives the services the patient needs. 21. Performs intake tasks as assigned. 22. Registers staff/patients with vendors such as pharmacy/EMR/supply company as directed 23. Assist with human resource functions as directed. 24. Distribute satisfaction surveys and compile results. 25. Type letters, reports and other materials as requested. 26. Answers the telephone in professional manner and channels the call accordingly. 27. Interacts with other disciplines as needed to ensure support. 28. Participates in Quality Assessment and Performance Improvement activities. 29. Completes all other duties as assigned/requested. Must have Hospice Experience! JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of chapters 246- 335 WAC and 70.127 RCW to the Washington Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of RCW 74.34.020 or 26.44.030 to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Authorizes and monitors the use of restraints when necessary and in accordance with the facility policies and procedures. Admits, transfers and discharges patients as necessary. Works with Unit Manager to help reduce hospital readmissions. Attends all-employee and departmental meetings and in-service training as scheduled or as directed. Quality, Compliance, and Risk Management - 20% Ensures that nursing procedures are followed by nursing service personnel, and that they reflect the day-to-day nursing care procedures used by the facility. Participates in the maintenance and implementation of the facility’s quality assurance program. Ensures the procedures for the administration, storage and control of medications and supplies are being followed by nursing staff. Ensures that resident rooms, treatment areas, work areas and equipment are maintained in a clean, sanitary, and safe manner. People Management and Development - 10% Supervises CNAs. Fosters a positive work environment that attracts, retains, and motivates employees. Interviews, selects, and orients new direct reports. Defines, establishes, and promotes performance expectations so direct reports understand and demonstrate excellence through high quality and consistent performance. Evaluates job performance of direct reports in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth. Monitors accountability with organization requirements prescribed by the Employee Handbook. Documents and engages employees who are routinely not observing these requirements in an effort to achieve compliance. Works with Human Resources when circumstances arise that necessitate further documentation and intervention. Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff. Ensures that there is a reasonable and sufficient plan for staffing should a direct report be absent or a direct reporting position be vacant.
