Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Responsibilities Provides professional nursing care to patients within unit. Maintain safe levels of care while adhering to a medical regimen. ESC is pioneering the first ever facility designed to care for young adults with severe and chronic medical needs who have aged out of pediatric services. Our future young adult center will be located on our White Plains campus and is slated to open in 2028. Reporting to the Administrator of the young adult center, the Chief Nursing Officer provides strategic and operational leadership to plan, organize and direct the daily operations in accordance with regulatory guidelines and standards. RESPONSIBILITIES Strategic Planning: Participate in ESC Strategic Planning Committee in partnership with the Board of Directors and be accountable to meeting the agreed upon metrics for monitoring success.
