Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. Collaborates with the physician and other health care providers to assure that a goal oriented care plan is consistent with medical regimen and hospital procedures. Responsibilities POSITION SUMMARY To ensure in collaboration with the Nurse Manager that each neighborhood provide safe, appropriate and goal-directed care and to provide support and direct nursing staff. To be familiar with all policies and procedures. Be able to seek appropriate guidance as needed. The RN Supervisor will be responsible for working an on-site schedule of a 3-4 days/week including weekends. Attends management meetings as required, this includes Operations Meetings and other Management Training as scheduled Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Follow-up on status of residents transferred to other facilities and reports follow up to oncoming shift, Team Leader and physician Communicates effectively with Center Departments regarding resident/family needs on tour of duty and documents communication in a clear and concise manner. Qualifications Education Requirements: Graduate of an accredited RN program B.S. N. (required); M.S.N. preferred RN in the State of New York BCLS certificate PALS certification preferred Completion of courses mandated for continuation of RN license, (ex. Infection Control course; Child Abuse course.) Experience Minimum of two years pediatric experience. Minimum of three years experience in supervisory role. Assists in applying safety restraints to residents when directed; Monitors residents in restraints, release restraints and documents according to policy and procedure Ensures that each resident who can use a call bell has one within reach and remind him/her to use it. Assists co-workers with lifting, positioning and other procedures as necessary. Be aware of facility policy on child abuse and/or neglect and promptly report any instances of abusive or inappropriate behaviors towards any resident to the NCC. Knows and follows The Center policy and procedure with regard to infection control including hand washing and types of isolation. Keep resident care area clean and free of clutter. Responsibilities The primary purpose of the Staff Nurse position is to be accountable for the care activities, safety and goal achievements of assigned patients through assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient’s response to the care. These responsibilities also include patient education and discharge planning. The Registered Professional Nurse places emphasis on mobilizing patients and preventing skin break down, falls and implements the appropriate measures to achieve this. The Registered Professional Nurse is a member of a designated care team and collaborates, coordinates and guides team members in planning patient care in a safe environment and achieving patient care goals according to the patient/family developmental stage and cultural expectations. Ability to lift and move patients with/without reasonable accommodations. Ability to organize care/activities of multiple patients. Successful completion of a Progressive Care Course or Progressive Care Certification, preferred but must be achieved within first six (6) months of hire. Phlebotomy Certification or IV Certification is preferred but must be achieved within first six (6) months of hire. Proficiency in the English language is required. FIT Testing required. Successful completion of a Critical Care Course or CCRN Certification preferred.

Responsible for assisting residents with daily care in accordance with established nursing care procedures. Liaison to nursing staff for communication of and/or routing of clinical care concerns. Qualifications: Administer medication as prescribed by the health care provider in congruence with the five rights of medical action administration. Provides assistance with dressing and/or appropriate clothing selection. Maintains an awareness of resident service plans and follows through by implementing care need changes promptly. Supervises and demonstrates pro-active care for resident on a daily basis. Assists residents with ambulation as needed. Advises nurse of concerns. Returns clean personal laundry to residents, takes care of residents' laundry and linen needs when necessary or when scheduled. Answers call bells promptly and courteously. Demonstrates understanding of how to approach, interact and respond to memory impaired resident. Demonstrates ability to deal positively with cognitively impaired resident. Transports residents within facility as needed. Reports change in Resident status, behaviors and other concerns promptly to nurse. Performs minor housekeeping tasks necessary to maintain orderly/neat appearance of the facility. Documents accurately according to facility policy. Notifies nurse on duty when any medication is not given as ordered. Receives/transfers oral and written reports at change of shift. Assists Activity Staff with daily activities (helps transport and/or encourages attendance, active assist when assigned.) Education: High school diploma or equivalent, preferred.
