Minimum one year of supervisory experience Responsibilities: Supervise all direct patient/family care team members, including nurses, social workers, hospice aides and counselors. Ensure that all patient/family services comply with federal, state(s) and local regulatory requirements and with accepted professional standards. Participate in the selection and evaluation of all direct patient/family care team members. Participate in the orientation and ongoing education of staff and volunteers. Assure that all initial assessments and required reassessments are conducted by qualified team members within required time frames. Coordinate the development, implementation and review of the interdisciplinary plan of care for each patient/family. Assure the 24-hour availability of nursing and psychosocial services to patients/families and coordinates "on-call" assignments. Position Summary: The delivery of quality care through the nursing process of assessment, planning, implementation and evaluation/re-assessment. The provision of direct and indirect patient care in the Telemetry Unit. Communicates with physicians about changes in the patient's clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision. Participates in performance improvement and CQI activities. Position Qualification: One (1) year acute care experience preferred Successful completion of Telemetry course/or experience in telemetry. Basic computer knowledge. Graduate from an approved nursing educational program as defined by the National Council of State Boards of Nursing (NCSBN) Required Licenses/Certifications: Current State (FL) Registered Nurse Licensure. American Red Cross Association BLS certification. Successful completion of Telemetry course/or experience in telemetry 
The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position.
