Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Communicates values, strategies, and objectives of department or division on a regular basis. Assigns accountabilities, delegates tasks and responsibilities, and plans, monitors and appraises job performance. 3. Develops, maintains and reconciles departmental budget with financial responsibility and oversight for department or division. 4. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes, or recommends changes to superiors as necessary. 5. In conjunction with Nursing, Medical and Quality Leadership, coordinates educational and performance improvement activities within a designated clinical specialty. 6. Develops and designs of action plans to support clinical performance and competency and provides support and educational resources to departments and services relative to nursing care delivery within the context quality assessment and improvement processes. 7. Establishes and implements short and long term goals, objectives, policies and operating procedures for high level of performance in nursing care delivery. 8. Coordinates and leads in the execution of day to day educational program activities, as appropriate to program objectives and areas of clinical expertise. 9. Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Baccalaureate Degree in Nursing is required. Masters in Nursing or health related field is preferred. Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. The salary range for the role is $64.8517 - $64.8517 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care. • Participates in the orientation of selected staff through ‘buddy system’ field observation of staff and feedback to appropriate staff involved in orientation process. • Participates in selected student programs by providing guidance, training and mentoring to students. • Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient/client care. • Transports and utilizes required medical equipment and other supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient/client homes/care facilities. • Travels to patient/clients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient/client. • Follows work procedures in relation to established workflows. • Maintains productivity sufficient to meet VNS Health goals. • Assumes responsibility for personal professional growth through attendance at in-services, training, continued formal education and select journal readership. • Performs other related duties and participates in special project, as assigned. • Establishes, implements, and revises a plan of care with input from appropriate multidisciplinary health team members (i.e., physician, OT, PT, SW, SLP, family members, caregivers, HHAs, etc.) based on goals mutually acceptable to the patient and significant others. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Participates in special projects and performs other duties as assigned.
