Most cases are colo-rectal, bariatric, urology, plastics, GYN, gender affirming, ENT, oncology and general surgical procedures. As a Registered Nurse on 8South you are responsible and accountable for the application of the nursing process and the delivery of patient care for the specialty patient population. You will provide administration of medications and therapeutic agents necessary to implement treatment, disease prevention, or rehabilitative plan of care. You will provide patient education on disease prevention and restorative measures. You will provide and accurately document direct and indirect patient care services that ensures the safety, comfort, personal hygiene, and protection of patients in a timely manner You will perform skin tests, immunizations, phlebotomy and initiate peripheral venous access. You will observe and assess signs and symptoms of illness, reactions to medications/treatments, general behavior, or general physical condition to determine normal versus abnormal characteristics and initiate emergency procedures when indicated. You will plan and implement individualized patient care based on observations. This will include appropriate reporting, referrals, and care in accordance with standardized procedures while providing care to special patient populations and patients with diverse cultural backgrounds. You will identify patients' readiness for learning and their ability to follow directions/instructions and give consent while identifying and assessing patient safety concerns with respect to age and developmental considerations. You will demonstrate the knowledge and the ability to identify and make special adjustments as required to needs of the specific populations. JOB DESCRIPTION Job Summary Provides medical assistant support for Care Connections team. Responsible for contacting members following appointments/services rendered to ensure member understanding and navigation of the health care system, and coordination support for follow-up care. Also provides assistance for other administrative clinical support tasks. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Coordinates care between members and providers after appointment/service delivery to support member navigation of the health care system, needs related to follow-up care and highest quality care/desired member health outcomes. • Communicates with Care Connections providers to clarify orders/requests and assists with escalation of issues to appropriate team(s) when unable to resolve independently. • Supports processing and follow-up for care management assignment and primary care provider referrals for high-risk members. • Escalates member issues related to prior authorizations for medications and durable medical equipment. • Communicates lab results and other exam findings with members and primary care providers. • Communicates with nurse practitioners on member health issues to ensure timely follow-up and response. • Supports care coordination and intervention by sending electronic messages/faxes to primary care providers. • Closes gaps in health care by preparing member lab result letters and coordinates follow-up on lab results. • Fulfills nurse practitioner supply orders and maintains adequate nursing supply inventory. • Performs administrative tasks such as telephone and email response, documentation follow-up and updates to member records. • Facilitates follow-up calls to members to ensure positive member experience/outcomes and ensure needs are met. • Assists with special projects under the direction of clinical leadership. Right at Home Long Beach is looking for an experienced CNA or LVN to provide hands-on, in-home training to our caregivers. This is a per-visit, hourly position — you'll be scheduled as needed to go into clients' homes and coach caregivers through the hands-on skills that are hardest to learn from a manual, with a particular focus on safe transfers and care for clients with limited mobility. The Caregiver Skills Trainer is responsible for creating extraordinary client and family experiences and ultimately trains our caregivers in the home. Provide one-on-one, in-home training to caregivers on safe transfer techniques, including Hoyer lift (mechanical/full body lift) transfers and gait belt transfers. Train caregivers on the care of bed-bound clients: safe repositioning and turning, use of draw sheets, skin and pressure injury (bedsore) prevention, and proper body alignment. Demonstrate and correct proper body mechanics to protect both caregiver and client during transfers, ambulation, and repositioning. • Coach caregivers on two-person transfers, sit-to-stand and pivot transfers, and wheelchair/bed-to-chair transfers. Train on personal care for clients with limited mobility: bathing, toileting/incontinence care, dressing, and oral hygiene in bed. • Review infection control and standard precautions (hand hygiene, PPE use, catheter and wound-adjacent care). Train on recognizing and reporting changes in condition — skin breakdown, signs of infection, falls risk, and changes in mental status. Observe caregivers performing skills in real time, provide constructive feedback, and document competency/training completion for the client's file. Communicate observations and concerns to the office and clinical team promptly. Model RAH's caregiving standards and represent the agency professionally in clients' homes. The desired candidate will possess the following: Active California CNA or LVN license/certification in good standing AND HCA required. Minimum 3 years of hands-on clinical or home care experience, including transfer techniques and mobility assistance.
