Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Capital Region (Albany-Schenectady-Troy) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow.
