Overview Shift’s Available: Days Employment Type: Full Time Hours: 8 hour days hsifts 8am-5pm Location: Southern Regional Medical Center – Riverdale, GA We are seeking an RN Case Manager, also referred to as a Registered Nurse Case Manager or RN Clinical Case Manager. An RN Case Manager will oversee and coordinate patient care from admission through discharge to ensure quality, cost-effective outcomes. They assess patients’ needs, develop care plans, and collaborate with physicians, nurses, social workers, and other providers to manage care transitions. RN Case Managers also conduct discharge planning, utilization review, and helping patients navigate insurance and healthcare systems. Responsibilities Patient Assessment: evaluate patients’ medical, psychosocial, and discharge needs upon admission and throughout their stay Care Coordination: develop and implement individualized care plans in collaboration with the healthcare team to ensure effective treatment and timely interventions Discharge Planning: coordinate safe and appropriate discharge plans, including referrals to rehab, home health, or long-term care facilities Utilization Review: monitor the use of hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays Patient and Family Education: help patients and families understand care plans, post-discharge instructions, and available resources Advocacy: advocate for patient needs and preferences, ensuring care is aligned with their goals and values Qualifications Education and Work Experience Starting April 1 2015. Minimum 5 years work experience post-graduation of an accredited school of nursing and a current state Registered Nurse license. Grandfathered prior to April 1, 2015. Minimum 5 years post graduate of an accredited school Of Social Work for Licensed Clinical Social Worker. However, RN Case Manager preferred. Five years acute care nursing experience preferred. Provides direct clinical services for critically ill adult patients within institutional guidelines and utilizing evidence-based clinical practice standards under the supervision of the patient's physician. 2. Obtains patient health histories and completes physical examinations and documents findings in accordance with hospital requirements. 3. Orders and interprets appropriate diagnostic and laboratory tests, explaining the necessity, preparation, nature, and anticipated effects of procedure(s) to patients, patients' family, staff, and healthcare learners. 4. Performs approved therapeutic or diagnostic procedures based upon the patient's clinical status and documents procedures in accordance with hospital requirements, 5. Performs cardiopulmonary resuscitation, airway management including bag and mask ventilation, endotracheal intubation, ordering life-supporting drugs according to established protocols, and defibrillation according to Advanced Cardiac Life Support standards. 5. Under physician direction and within the credentialing standards of the Hospital, inserts and manages arterial catheters, dialysis catheters, central venous catheters, pulmonary artery catheters, chest tubes, thoracenteses, endotracheal tubes, and extraglottic devices. 6. Interfaces with patients using monitoring and instrumentation equipment, and directs and educates others in performing these functions. 7. Formulates and implements a treatment plan for acute illness management in collaboration with the attending physician(s) and documents in medical records and appropriate narrative summaries. 8. Assesses patients for changes in status and institutes appropriate interventions, including initiation of necessary emergency interventions to stabilize the patient. 9. Incorporates clinical research findings in the development and implementation of standards of care. 10. Supports IRB-approved clinical research/trials through a variety of activities, including but not limited to: obtaining consent; ordering diagnostic tests; recording laboratory and diagnostic results; administering therapeutic interventions; and documenting and reporting patient outcomes, including toxicities or adverse events. 11. Performs and participates in quality/performance improvement activities and clinical research. 12.
