Core Responsibilities: Support patient assessment and prioritization for both walk-in and telephonic triage to ensure patients receive the right level of care and avoid unnecessary hospital visits. Execute protocol-driven chronic condition management. Perform skilled nursing procedures including wound care and medication administration (including vaccines). Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care in person, over the phone, or in group settings. Conduct medication reviews within nurse-led visits to ensure patient safety and adherence. Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice. Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care. Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic. Utilize standardized protocols for medication management, prescription renewals and prior authorizations. Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner. Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency. Performs other related duties as assigned. Description The Per Diem Clinical Oncology Nurse Navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge, who provides individualized assistance to the patients, families, and caregivers through informed decision making, timely access to quality health and psychosocial care, and coordination of appropriate follow-up visits throughout the cancer continuum. This individual helps overcome healthcare system barriers, influence the systems and behaviors, and address the gaps of care through research and evidence-based practices guided by regulatory committees and professional organizations. The Per Diem Clinical Oncology Nurse Navigator supports the organizational strategies to deliver high-quality care to the population served through community needs assessment, navigation program development, marketing and outreach, and ONN specific-outcome metrics (i.e., care coordination, guidance, education, and advocacy) established by the organization. The Per Diem Clinical Oncology Nurse Navigator initiates, develops, and implements mechanisms for effective collaboration with the interdisciplinary team, that includes (but not limited to), new patient coordinators (NPC), clinical oncology associates, clinical coordinators (RNs), clinical infusion nurses, advanced practice providers (NPs & PAs), physicians, social workers, and nursing/administrative leadership for the attainment of the highest standards of patient care. Description Patient Care Associate-Nursing 9e Telemetry - STL- Mount Sinai Morningside- Full Time- Evenings 3pm-11pm- Rotating Weekends As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting.
