The responsibilities of this job include, but are not limited to the following: Locating patients, obtaining accurate vital signs by visual and auditory measures, triaging patients, assessing patients’ medical condition, and preparing patients for physicals and medical treatment by Physicians and Nurses; Understanding and using the electronic medical record system; Ordering lab, x-ray, and medical supplies as directed; Notifying the Nurse and/or Physician when the patient is ready for exam while communicating symptoms and relaying questions about lab or x-ray needs; Assisting Nurses and Physicians with various forms of patient treatment and care; Handling Physician and Nurse requests with respect and in a timely manner; Performing lab duties as assigned; Performing phlebotomy procedures; Sterilizing and setting up instruments; Receiving and storing supplies; restocking treatment rooms as needed; Reporting to the Charge Nurse before leaving at the end of the shift; Following ancillary guidelines as deemed necessary by the Charge Nurse, Nurse Supervisor, DMS, and Physician; Adhering to established policies and procedures; Maintaining cleanliness of the treatment area and completing biological cleanup in accordance with OSHA guidelines and other maintenance assignments as directed; Washing, drying, folding, and restocking laundry; maintaining the cleanliness of the washing machine and dryer lint trap; Attending staff meetings as scheduled by the Nurse Supervisor or DMS; Operating, using, and maintaining medical and office equipment as trained; Completing assigned checklists and Policy Manager tasks within the assigned shift; Being available to assist as needed, as breaks and mealtimes may be interrupted at any time to provide necessary patient care or to maintain center operations; Fostering teamwork and ensuring a positive and professional atmosphere; Providing positive, warm, and friendly service in all interactions; Understanding and performing all duties as assigned. Minimum education and professional requirements include, but are not limited to, the following: Must be 18 years of age or older; Minimum typing experience; High school graduate or equivalent; Minimum of one year of clinical experience in a hospital or physician’s office preferred; Ability to hear pages, bells, and phone system; Ability to sit, stand, and walk for long periods of time (possibly 4 to 7 hours at a time); Ability to lift up to 50 pounds; Ability to attend scheduled CPR classes and code drills; Ability to prioritize and multitask; Willingness to work at any center due to a staffing or center emergency or reduction of work.

Under general supervision, acts as palliative medicine consultant in various settings, including hospital, nursing facility and home. Makes recommendations for care plans, symptom management and goals of care. Works in collaboration with staff and attending physicians within care settings.Education: Successful completion of a Nurse Practitioner program; Master’s in Palliative Care preferred. Licensures/Certifications: Current registration with the Maryland State Board of Examiners of Nurses as a Nurse Practitioner, specializing in Adult, Family or Geriatrics; Inpatient position can include Acute Care certification. Schedule: 24 hours weekly, 7a.m. - 3 p.m. and 3 p.m. - 11p.m. with rotating weekends. Type: In Person The Adult Crisis Stabilization Unit is a 22-bed unit offering a highly focused crisis intervention model for the treatment of major mental illnesses where the average length of stay is less than 5 days. As a Registered Nurse/RN you will: Perform a full range of professional psychiatric nursing services, including assessing, planning, administering, and evaluating patient care in accordance with team treatment plans. Administer medical treatment as prescribed by physician. Teach and provide psychosocial support to patients and their families. Provide day-to-day clinical direction to assigned Mental Health Workers or other paraprofessional staff. Document and report patient progress to treatment team. Requirements: Must possess a current license as a Registered Nurse (RN) in the State of Maryland (or compact state) at time of appointment and continuously while in position. Experience in a psychiatric nursing setting preferred. Comprehensive knowledge of nursing-acquired through at least one year of inpatient RN experience; prior LPN inpatient experience will be considered with every one year of LPN experience equating to 6 months RN experience Must be able to work at least four shifts per month All new clinical staff will begin their extended clinical orientation/training immediately following your first day of employment. The pay range for this position is $37.08 minimum to $56.83 maximum. Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.

Coordinates patient, family, significant other, and health team members in the planning of hospital and post-discharge care. C. Implementation. 1. Performs clinical nursing skills independently. 2. Administers medications, IV therapy and treatments appropriately and accurately. 3. Initiates appropriate emergency procedures as necessary. 4. Appropriately delegates and coordinates the duties of ancillary personnel. 5. Communicates the patient's status and plan of care through written documentation in the patient's medical record. 6. Principal Duties and Responsibilities Assumes responsibility for the initial evaluation and treatment of patients in Gilchrist Inpatient Centers Performs initial Hospice history and physical as evidenced by appropriate documentation. Writes standard orders in accordance with hospice policy. Writes appropriate orders related to pre-existing conditions. Discusses with the attending physician and nursing staff physical and/or emotional findings, which may interfere with care of the patient. Manage all aspects of the patient’s symptoms, including medication/prescription management, conduct family meetings as needed and requested, coordinate care with other members of the IDT through daily rounds, and document care activities and the rationale for the GIP level of care. Responsibility for completion of the MOLST form as required and for the completion of the death certificate of each expired patient. Attend the weekly inter-disciplinary team meetings Work with the hospice access team to facilitate a smooth admission process. Work with the Gilchrist Inpatient Centers inter-disciplinary team to facilitate live discharges from the Center, including completion of transfer/discharge summary, prescriptions, and other discharge related documentation. Provide telephone and afterhours coverage via emedicall for patients at the assigned Center Evaluates patient progress on a continuing basis and in collaboration with the attending physician, and alters treatment modalities as needed. Evaluates nursing, medical, social and educational needs on assigned patients. Writes progress notes for each visit documenting pertinent information regarding patient progress and treatment regime. Writes orders appropriate to the patient's care requirements in compliance with existing contract. Establishes medical diagnoses in compliance with contract. Refers to the physician or designee those problems which require consultation and documents same on progress note. Performs technical functions as covered by the job description and delegation agreement submitted to BPQA and/or MBON, approved NP agreement and in accordance with hospital policy. Documents results of technical functions performed. Reports to physician or designee any abnormal findings related to it. Monitors patient response related to technical function performed and documents it. Acts as a clinical resource. Actively supports and participates in nursing and/or hospital performance improvement initiatives. Leads staff wellness activities as directed by supervisor. Performs activities to meet patient/family educational needs. Participates actively in health teaching of patient and family to include preventative health care as well as current medical condition and treatment regime. Evidence of same by documentation on progress note and/or established flow sheet. Plans and reviews home care instructions with patient and family if needed. Include needs for service provided by community resources and hospital based ancillary services. Educates and supports patient and family using integrative/palliative principles to facilitate patient and family well-being Assumes responsibility for utilizing opportunities for educational advancement by pursuing activities to meet identified educational needs and those required for continue certification as well as attending mandatory in service as required. Job Requirements The Care Manager coordinates the care and service of selected patient populations across the continuum. He/ she works collaboratively with physicians and other members of the health care team to achieve the highest quality clinical outcomes with the most cost-effective use of available resources. Participates in the hospital Ethics Committee, as well as facilitates the hospital UR committee. DOCUMENTATION AND MEASUREMENT: Uses established criteria to determine appropriateness of admissions, transfer and continued stays. Documents actions in medical record according to departmental guidelines and oversees process of exchange of information with other facilities/ agencies adhering to legal mandates regarding confidentiality. Measures effectiveness of interventions through direct communication with patients and caregivers and data collection of defined indicates (e.g. overall length of stay, readmission rates, feedback from referral services, etc.). PATIENT CARE: Uses severity of illness/ intensity of services indexes to determine appropriateness of admissions, transfers and continued stays. Provides physicians and ancillary departments with data on treatment outcomes and avoidable delays in order to promote highest quality care. Identifies potential areas of compliance vulnerability and risk; develops and implements corrective action plans for resolution of problematic issues; and provides general guidance on how to avoid or deal with similar situations in the future. Measures effectiveness of interventions through direct communication with patients and caregivers and data collection of defined indicators (e.g. overall length of stay, readmission rates, feedback from referral sources, etc.). Plays and active leadership role in the ongoing management of inpatient length of stay and utilization review across the organization.
