On the day-to-day, you will: Conduct home health nursing assessments to determine the nursing equipment needs in the home, including participant preferences and goals Actively participate in IDT meetings to develop participant care plans Deliver and document home health nursing interventions as agreed upon in participant care plans, including but not limited to maintaining a healthy and safe environment, promptly responding to physician orders, and accurately administering medications and performing ordered tests and treatments Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions Track and monitor home health scheduling in collaboration with the Home Health Manager or scheduling point of contact Collaborate with the Physician and other IDT members to assist in smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.) Job requirements include: Graduate of an accredited school of nursing Unencumbered RN License required Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred Reliable means of transportation Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred We are seeking Home Health RNs that ideally have experience working with frail and elderly populations. On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life!
