Summary: The Clinical Nurse 1 New to Practice/Apprentice is a newly licensed new-to-practice RN or RN with less than 15 months of experience that demonstrates basic safe practice within the organizational professional practice model. The Clinical Nurse 1 New to Practice/Apprentice seeks guidance to integrate concepts, knowledge, skills and attitudes to meet standards for competent professional nursing practice as specified in the Clinical Nurse I New to Practice/Apprentice position description addendum. The Clinical Nurse 1 New to Practice/Apprentice exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. The Clinical Nurse 1 New to Practice/Apprentice is accountable for the provision of direct care to patients who may range in age from infancy to the elderly and demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Teamwork, and Technology/ Informatics (Definitions listed below). The Clinical Nurse 1 New to Practice/Apprentice works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on developing the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients. Responsibilities Nurse Manager, works in conjuction with Hospice Executive Director and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Nurse Manager in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Nurse manager will be on call according to scheduling. During the hours when on call, the Nurse Manager is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Hospice Executive Director.

Reports relevant information to the DON, Assistant Director of Nursing (ADON), and Unit Manager including, interdisciplinary team functioning and communication, regulatory compliance issues, resident risk factors, sadden changes in resident behavior, expressed dissatisfaction by resident or family, refusals of treatment, unexplained injuries, staff behavior (positive and negative), staffing needs, equipment and supply needs, opportunities to revise policies, procedures, improve quality. Assists DON and ADON with preparation for long-term care survey. Attends survey training, interacts with state surveyors as instructed by supervisor. Education/Training/Work Experience: Education: Graduate of accredited school of nursing, BSN preferred Certificate/Licenses: Current, active license as a Registered Nurse in Pennsylvania or current Multi-State license. Active CPR Certification required Work Experience: Two years prior experience as a Nursing Supervisor required. QUALIFICATIONS: Graduate of an accredited college or university with a Bachelor’s or Master’s Degree in Nursing Currently registered in New Jersey, by state examination and experienced in supervision and teaching. POSITION SUMMARY: Performs nursing services based on the facility policies and procedures as regulated by the State and Federal law. PART 1: JOB RESPONSIBILITIES AND STANDARDS PERFORMANCE 1. Responsible for direction, provision and quality of Nursing care. a. Provides guidance and training to ADON and DON’s in training. b. Provides weekly supervision to ADON’s to discuss and review key issues. c. Provides support to Nursing Leadership through weekly meetings. 2. Responsible for ensuring one registered professional nurse is on duty at all times and reviews monthly Nursing schedules. 3. Reviews nursing 24 hour reports and brings issues for resolution to the daily morning meeting. 4. Makes every effort to adhere to Nursing Department budget as developed by administration; supervises ordering of medical supplies. 5. Responsible for coordinating and integrating nursing services with other patient care services to provide a continuum of care for the patients. 6. Participates in representing the Nursing Service in the facility’s staff committees including Patient Care Policy, Pharmacy, Infection Control, Utilization Review and CQI and Safety. 7. Responsible in the development and periodic update of written job description for each level of Nursing Personnel. Reviews and signs competency evaluation of personnel. 8. Ensures completion, documentation of Incident/Accident Reports in a timely manner. 9. Ensures that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status. 10. Assists the administrator in the enforcement of Resident Care Policies. 11. Works as a team member with all other departments/auxiliary services 12. Prepares Department of Nursing for State Survey through monitoring of CQI data and systems in place. 13. Provide direct Supervision to companywide MDS 3.0 Coordinators in all buildings to ensure accuracy and timeliness of assessments. 14. Provides In-Service training to Nursing in accuracy, scoring and assessment as it relates to MDS 3.0. 15. Monitors compliance with Monthly Pharmacy Consultant Reports, Monthly Psychoactive Summary Reports and Monthly Pain Management Summary Reports 16. Develops plans of corrections and training to all MDS Coordinator based on outcome of QA audits. PART II. FACILITY-WIDE RESPONSIBILITIES & STANDARDS 1. Demonstrate courtesy to resident, their families, and co-worker. 2. Maintains resident confidentiality. 3. Demonstrates ability to work with others. 4.
