The Patient Care Partner may be assigned to Emergency Department or inpatient units to meet the unique needs of this population of patients. About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. Essential Functions: Responsible for medication administration for the residents in compliance with Federal & State regulations Assist with admissions of new residents, including assessments and documentation Assist with the process of implementing acute and long-term care plans or service plans an/or the collection of data Responsible for clinical system tracking (i.e., wounds or infection control) under the director's direction Provide general consultation regarding health-related concerns within the community Notify physicians and/or family members of any change in the resident’s health and provide proper documentation Respond quickly to all emergencies Maintain close communication with all departments and staff members Provide educational meetings for residents and staff Understand the role of the Safety & Disaster Plan Maintain confidentiality of residents’ and prospective residents’ information Attend all required training, in-service, and staff meetings Maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers Perform other duties as assigned Qualifications: License/Certification: Current state license as an LPN and CPR certification Graduate of an approved LPN program (per state requirements) Minimum of 1 to 2 years of experience working as a nurse preferred Previous experience working with the elderly in a paid or volunteer position is highly desirable Job Type: Full-time, Part Time, Per diem Salary: $32.00 - $34.00 per hour Benefits: 401(k) Dental insurance Free parking Health insurance Life insurance Paid time off Paid training Vision insurance Medical specialties: Geriatrics Ability to commute/relocate: Clinton, CT 06413: Reliably commute or planning to relocate before starting work (Required) License/Certification: LVN (Preferred) BLS Certification (Preferred) LPN (Required) Work Location: In person

Job Opportunity: LPN Charge Nurse Starting Pay: Competitive rates up to $37 per hour, based on experience Differentials Offered: Weekend Differential: $1.50 for hours worked on Saturday and Sunday Additional Shifts Differential: Extra $5.00/hour for any shifts worked outside scheduled hours up to 40 hours. Available Shifts: 7a-7pm (Mon-Friday hours available) 24 Hour part-time and 8 Hour shifts 3-11pm On-Call Responsibility Responsibilities : The LPN Charge Nurse plays a pivotal role in planning, executing, and overseeing nursing services for assigned residents. This includes adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions. Key Duties: Efficiently plans and provides professional care while aligning with the needs of residents and the changing medical landscape. Maintains professionalism in interactions with residents, physicians, families, peers, and support staff. Documents crucial information related to resident care accurately and comprehensively. Administers prescribed medications and oversees accurate record-keeping in line with facility policies. Requirements: Valid state nursing license in good standing (or in the process of becoming a registered nurse). In-depth knowledge of nursing practices, medical procedures, and long-term care regulations. CPR certification required Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively. Collaborative team player with previous experience in long-term care preferred.
