Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, Joint Commission, and State & federal regulations. Reports to Nurse Manager. CLINICAL PRACTICE - On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. POPULATION SPECIFIC - Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. UNIT LEADERSHIP - Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. DOCUMENTATION/COMMUNICATION - Completes all required documentation in a timely manner, according to CCMC policies and standards; necessary information is effectively communicated to all members of the health care team. PATIENT/FAMILY PARTNERSHIP & EDUCATION - Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. CCMC STANDARDS - Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CCMC Standards. Education and/or Experience Required: A minimum of ASN/ADN required upon hire, BSN preferred. If ASN/ADN, must acquire BSN within 3 years of hire. Minimum of 2 years recent perioperative experience. License and/or Certifications Required: Current State of Connecticut DPH Registered Nurse licensure OR current multistate nursing license under the Nurse Licensure Compact (NLC) required and maintained thereafter. American Heart Association, or American Red Cross, Healthcare Provider CPR/BLS certification required upon hire and maintained thereafter. Licenses and/or Certifications Preferred: Certification in OR nursing is preferred. Pediatric Advanced Life Support (PALS) required within 6 months of hire. (Traveler that does not do call does not need PALS certification). Knowledge, Skills and Abilities: Knowledge of: Theory, principles, techniques and practice of professional nursing. State regulations regarding nursing practice. Clinical standards of care; documentation and patient record policies and standards. Teaching techniques; identification of learning needs and provision of patient/family education. Developmental and population-specific patient & family assessment techniques and how to apply to practice Accreditation and certification requirements and standards. Skills: Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Basic proficiency in utilization of personal computers in networked Windows-based health care environment to access related software applications; locate, open, edit and print files and information utilized by the unit; and use of Internet as research tool. Provides nursing leadership in order to meet the goals of the ARK organization and the individual center. Completes rounds on the units to observe residents and to determine if nursing needs are being met. Provides patient care when required. Monitors nursing care to ensure positive clinical outcomes and minimize rehospitalizations. Ensures Point Click Care (PCC) is utilized according to the Business Processes. Encourages communication between licensed nursing staff and CNAs during and between shifts. Encourages shift-to-shift communication between incoming and outgoing nursing staff. Ensures timely and accurate compliance of the MDS process and that it reflects the residents’ plan of care. Reviews prospective admissions in relation to existing nursing capabilities and ensures readiness to treat. Contacts attending physician to obtain orders as indicated (i.e., admission, change in condition, etc.). Ensures that Physician Orders are followed as prescribed. Oversees medication management to ensure adequate supplies and that all medications are handled in accordance with ARK policy. Ensures that resident’s attending physician and family or responsible party are promptly notified of any significant change in the residents’ health condition. Facilitates discharge planning process and provides consultation as needed. Investigates resident/family complaints and takes appropriate actions to bring to resolution.

Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner.
