Overview The primary objective of this role is to enhance patient outcomes through effective chronic care management while driving improvements in departmental quality metrics. Key focus areas include optimizing Medicare Wellness visit compliance, closing patient care gaps, and ensuring timely completion of transition of care processes, including hospital follow-up coordination. Responsibilities Schedule Medicare Wellness Visits in compliance with frequency guidelines set by CMS and individual insurers. Scan upcoming appointments for additional medical record information necessary. Obtain medical records from other physician offices, hospitals, labs, emergency rooms, and radiology departments prior to or following patient visit. Follow chronic disease and preventive care guidelines and ensure that appropriate plans for completion are developed. Complete Medicare Wellness Visits meeting a set weekly quota, documenting all required components, addressing necessary health maintenance gaps in care, and develop individualized patient specific care plans. Appropriately document all nursing interventions in the electronic medical record. Review the chart for completeness, test results and billing. Utilize Excel to maintain an accurate lists of patients eligible for Medicare Wellness Visits, patients with completed visits, and document outreach to patients Follow transition of care management/hospital follow up workflow completing 48-hour transition of care outreach to patients coordinating care to ensure all discharge needs are met, and arranging hospital follow up appointments for patients. Provide chronic care management services to patients collaborating directly with SACO/SQCN care management team members to improve patient medication adherence, fulfill DME requests, assist with obtaining referrals, and coordinating care with specialists’ office. Identify patients who would benefit from additional care management support and place referrals/orders for necessary services. Complete all Epic in basket requests appropriately. Administer injection of medications or immunizations in accordance with approved nursing protocols. Observe patient for any reaction to medications or immunizations, treatments and significant incidents. Attend Care Management Meetings, Primary Care Collaborative meetings, and other educational sessions as scheduled. Will complete recommended educational modules as assigned. Assist in clinic with rooming and triaging patients as needed. Provide basic patient care as ordered. Respond to life threatening situations based upon nursing standards, polices, procedures and protocols. Qualifications Required: Ability to manage time effectively and work independently to complete all assigned tasks. Ability to work with other team members as part of the interprofessional collaboration team Ability to make professional nursing decision up to the scope of practice for licensure and possess the knowledge and critical thinking skills to support those decisions. Overview This is a position for a Medical Assistant with the Department of Family Medicine. Under direct supervision this position will assist with clinical duties and provide direct patient care, including taking vitals, administering meds/treatments, assisting physicians, doing wound care, patient education, documentation, and coordinating care, labs, and follow-ups in a fast-paced office. Responsibilities Triage incoming patients by obtaining vital signs, following standing triage orders, reconcile medication list, reconcile allergies, make sure immunizations are up to date and prepare patient for the visit. Completes delegated tasks from the current and prior day i.e.: patient messages, prescription refills, lab results, referrals, home health agency requests, medication prior authorizations, patient forms, and physician follow-up. All tasks must be reviewed, addressed, and marked appropriately before the close of each business day. Assist physicians with examinations, minor surgeries, diagnostic procedures and treatments. Provide basic patient care and education as directed by provider and standing orders. Coordinates services with specialists i.e.: radiation oncology, pulmonology, endocrinology, nutritional counseling, or social services consultation. Overview The Lead Medical Assistant (Lead MA) in the Minimally Invasive Gynecological Surgery Division provides advanced clinical and administrative support while serving as a leader and resource for medical assistant staff. This role ensures efficient clinic operations, promotes high-quality patient care, and supports departmental goals through coordination, training, and collaboration. The Lead MA functions both as a clinical expert and as a liaison between providers, staff, and leadership to enhance workflow, patient satisfaction, and operational excellence. Responsibilities Leadership, Training, and Operational Oversight – 35% Participate in the implementation of Department and EVMS Medical Group policies and procedures. Promote the philosophy, goals, and objectives of EVMS and the Department of OB/GYN. Collaborate with department leadership to maintain efficient clinic workflows. Serve as a designated Electronic Health Record (EHR) Super User for providers and staff. Oversee completion and processing of clinic documentation. Provide daily end-of-shift reports to department management. Perform regular audits, included but not limited to chart audits and procedure audits. Train and onboard new clinic staff on triage, workflows, tasks, and documentation. Monitor clinic operations and address workflow needs proactively.
