ESSENTIAL DUTIES & RESPONSIBILTIES: By following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following: • Maintain a healthy, safe environment. Providing nutritional and environmental support. • Assist with personal hygiene, toileting, dressing, walking and feeding the client. • Position the client when instructed to do so. • Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor. • Remind the client to take medications. • Assist with payment of bills ONLY when authorized to do so and it is documented on the assignment sheet. • Run errands when authorized to do so. Documentation on the Aide Charting Sheet daily. • Report any important information or changes in the client's condition to the agency and/or the nursing supervisor when necessary. • Use of medical supplies and equipment when appropriate. • Emotional support and the development of a trusting relationship between the aide and the client. • Encouragement of client independence. QUALIFICATIONS/EDUCATION: • Must be eighteen (18) years of age or older. • Must have a certificate from a Home Health Aide training program consistent with State requirements with evidence of ongoing in-service education to maintain the certificate. • Must have the maturity necessary for establishing and maintaining a good work relationship with the client, the client's family and agency personnel. • Have a sincere interest in and a sympathetic attitude towards caring for the sick and disabled. A HOME HEALTH AIDE MUST: • Get along well with others. • Be dependable and trustworthy. • Maintain compliance with all agency policies and procedures. • Maintain in-service requirements as specified in State and agency regulations. Participates in after-hours, weekend, and holiday coverage as needed. 14. May perform other duties as assigned. Qualifications EDUCATION Nursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred. EXPERIENCE Minimum two (2) years of nursing experience in an acute care setting. Previous home care experience is desirable but not required. LICENSURE Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire. SPECIAL SKILLS Excellent clinical knowledge, verbal/written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team. PHYSICAL DEMAND Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Description A Nurse Practitioner is certified under section 6910 of the New York State Education Law and licensed as a Registered Nurse. Provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1.Responsible for patient care as outlined within collaborative practice agreement. 2.Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 3.Assesses the health status of patients/families by means of comprehensive health history and physical examination. 4.Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area. 5.Provides teaching and counseling in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 6.Documents findings and plan of care in the patient's medical record. 7.Consults with physician as established in the collaborative practice agreement. 8.Initiates referrals to specialty services and fosters continuity of care. 9.Participates in team meetings and conferences to enhance an interdisciplinary approach to acute/primary health care delivery. 10.Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months 11.Supports the mission, vision, philosophy and goals of the Department of Nursing and the Medical Center. 12.Demonstrates accountability for own nursing practice. 13.Advocates for patients/families and supports patients' rights. 14.Considers ethical issues of professional nursing practice and adheres to the code of ethics. 15.Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. 16.Serves as a resource person to nurses and other health team members. 17.Participates in peer review. 18.Incorporates research findings in clinical practice. 19.Participates in approved nursing and medical research studies. 20.Keeps knowledge current by self directed learning, literature review and attendance at educational programs. 21.Participates in defining, maintaining, and interpreting standards of nursing practice. 22.Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. 23.Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. 24.Seeks opportunities to enhance professional nursing practice and influences outcomes. 25.Treats co-workers, patients and families with dignity and respect.
