In this role, the successful candidate Responsible for using own clinical expertise to provide breastfeeding education and counseling. The Lactation Coordinator works collaboratively with other health team professionals to promote breastfeeding management. The Lactation Coordinator provides leadership to the patient care team by mentoring and sharing current evidence-based information and clinical skills. Job Responsibilities: Manages department utilizing resources in fiscally responsible manner. Establishes reasonable budget projections for supplies, effectively manages fiscal responsibilities related to salaries and supplies. Prepares budgets and related reports in a comprehensive and timely manner, within established financial guidelines. Works to provide adequate staffing levels on each shift. Models an open approach to information/knowledge sharing by providing interactive forums including monthly scheduled staff meetings including minutes and attendance sheets. Ensures staff participation in annual requirements. Maintains clinical knowledge base appropriate to patient population. Maintains competencies; participates in activities/meetings/committees contributing to the growth and development of self, colleagues and the nursing profession. Obtains a minimum of 20 CEUs annually relative to specialty and role. Perform other duties as assigned. Establishes, monitors, and evaluates standards of patient care specific to the individual patient care unit focusing on measurable outcomes. Consistently addresses patient care needs in a timely manner. Participates and/or promotes multidisciplinary process improvement activities for unit-specific and hospital-wide initiatives. Actions demonstrate an understanding of individuals role and how it relates to the delivery of care/services in the organization. Demonstrates accountability with respect to staffing and scheduling. Practices, facilities and role models the principles of team building, delegation and communication. Assists staff with developmental plan for professional growth. Oversees the orientation process of new employees including unit based and float pool staff. Maintains employee unit files as per WUH/HR guidelines. Minimum Qualifications: To qualify you must have a Bachelor's Degree in Nursing Required. Current NYS Registered Nurse License Required. Duties & Responsibilities Purpose of Position : Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being. Examples Of Typical Tasks Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients. Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients. Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. Participates in defining, implementing, maintaining and interpreting standards of nursing practice. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. Participates in determining conditions, resources and policies essential to the delivery of nursing care services. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Performs other professional nursing activities as delegated by nursing service administration. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Essential Duties & Responsibilities of Licensed Practical Nurse (LPN): Assess and document resident progress in the medical record. Monitor turning and positioning of residents. Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated. Maintain clean, orderly treatment cart and restock treatment supplies. Follow Infection Control policies and procedures. Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures. Document pressure sore status weekly or more often as condition changes. Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents. Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home. Participate in Quality Improvement activities, as assigned. Job Requirements of Licensed Practical Nurse (LPN): Licensed Practical Nurse current state license in New York. At least one (1) year of Skilled Nursing experience preferred. Must have a current/active CPR certification. Excellent technical, assessment, and documentation skills. Effective verbal and written English communication skills. Customer service oriented with the ability to work well under pressure. Ability to work with minimal supervision, take initiative and make independent decisions. Approachable, flexible and adaptable to change.
