How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Overview About the Role The Oncology RN Clinical Care Coordinator (OCC) works collaboratively with an oncologist in delivering care to their assigned patient population. This is an outpatient position and requires oncology clinical experience and sound nursing knowledge related to the care of patients undergoing cancer treatment to assist with the education, the coordination of care, and ongoing assessment of the patient's response to treatment. The OCCC assesses patients for symptoms, clinical and psychological needs, treatment-related concerns, and barriers to care. Provides education regarding diagnosis, treatment, medications, side effects, and symptom management. Connects patients and families with appropriate resources as determined by the healthcare team. Functions as the primary clinical point of contact for hematology and oncology patients and follow them throughout the continuum of care. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases.
