PURPOSE AND SCOPE: The professional registered nurse Inpatient Services RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. · Performs ongoing, systematic collection and analysis of patient data pre – during – post treatment for assigned patients and documents in the patient medical record, makes adjustments and modifications to treatment plan as indicated and notifies Supervisor, Physician, patient’s primary nurse and others as may be indicated. · Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input including but not limited to Supervisor, appropriate physician, and the contracted facility primary Nurse. · Directs and provides safe effective patient care for assigned patients as prescribed for all modality specific treatment procedures. · Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed. · Initiates and coordinates communication with FKC and Non-FKC dialysis providers and appropriate contracted facility personnel as needed to provide continuity of patient care. · Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. · Administers medications as prescribed and documents appropriate medical justification and effectiveness. · Initiates and assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · May be asked to provide specialized nursing care instruction to hospital/facility staff as stipulated contractually. · May be assigned to assist in an Outpatient facility on an as needed basis. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. PRINCIPAL DUTIES AND RESPONSIBILITIES: QUALITY: Responsible for supporting and driving organization quality standards through meeting regulatory and accrediting body requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools. Assists with implementation of organization quality goals and facility specific action plans in order to achieve organization quality standards. Position Overview: The Weekend Supervisor is responsible for directing the operation of a nursing unit in a long-term community. Key Responsibilities: Perform general management functions such as hiring, disciplining, and evaluating care team members. Plan and facilitate meetings and committees to address resident care issues/concerns for the unit. Manage area of responsibility with the goal of achieving and maintaining the highest quality of care possible. Participate in developing, implementing, and evaluating programs that promote the recruitment, retention, development, and continuing education of nursing staff members. Contact physicians as needed/necessary and ensure physician interventions are timely and appropriate. Review staffing patterns and census of nursing units and reassign personnel when necessary to ensure staffing meets resident needs and budgetary controls. Monitor the clinical operations of the unit and resident’s conditions and ensure that appropriate and quality care is administered. Obtain medications, supplies and medical records needed to provide safe, efficient, and therapeutic are to residents on a continuing basis. Assist in the orientation of new personnel, monitoring their skills and guides and observe care team members that may need assistance with procedures. Qualifications: Graduate of accredited school of nursing. Licensed as an RN in the state employed; license must be active, valid, and in good standing. Valid CPR teaching certificate preferred, but not required. Must hold an active CPR license or the ability to obtain within the first 30-days of employment. CPR license must be kept active and current throughout employment. Three years of nursing experience, of which one year in a long-term care environment. This unit is uniquely designed for individuals whose behavioral health challenges, combined with acute or chronic medical diagnoses, make them difficult to place in traditional psychiatric or medical settings. Patients on the IMAP unit may present with a wide range of medical concerns, including post-surgical recovery, cardiac abnormalities, sepsis, wound care, or other acute conditions—while also experiencing psychiatric diagnoses such as schizophrenia, bipolar disorder, or severe depression. The unit's structure allows for close medical monitoring while simultaneously addressing behavioral health needs through a multidisciplinary team approach. With a nurse-to-patient ratio of 1:4 / 1:5 and a CNA ratio of 1:6, the unit is staffed to support safe, effective, and compassionate care. The team on 5A delivers respectful, patient-centered care to individuals with complex access and care needs, and works collaboratively with internal medicine, psychiatry, and social work to ensure holistic treatment.
