Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Aurora and surrounding locations. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
