Job Description The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. A MSN or DNP are not required but supersede the requirement for the BSN. ACCU cares for patients ages newborn to adults, including those with heart failure, pulmonary hypertension, heart and multi-organ transplants, post-procedural care, and congenital heart disease. Therapeutic modalities include noninvasive monitoring, cardiac telemetry, oxygen therapies, feeding tubes, central line access, inotropic/anticoagulation/antiarrhythmic drug therapy, NIRS, and complex drainage tube management. Patients may have additional specialty mechanical pumps, including mechanical circulatory support devices such as the Berlin Heart, the SynCardia total artificial heart, and the HeartMateIII. While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population. We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities. NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of KY Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care.
