University Center is hiring a Licensed Practical Nurse (LPN) for our Skilled Nursing Facility in Bronx, NY. We are looking for an LPN to work 2 shifts a week on our overnight shift Now Offering a $2,500 Sign-On Bonus Base rate is $33.54 with an additional 10% shift differential for evenings and nights. Shifts: 11PM-7AM We now offer Daily Pay through a trusted third-party provider giving you faster access to your earnings!! DUTIES: Collecting required information from new Residents to be admitted. Recording health details of Residents; including vitals & temperature. Administering medications and injections to Residents as needed. Treating and dressing wounds and bedsores as needed. May be required to supervise Certified Nursing Assistants (CNAs). Helps Residents get dressed & take care of personal hygiene. Monitors Residents’ food and liquid intake and output. REQUIREMENTS: Must be able to work as a team member. Valid LPN State license. Must be in good standing with State Registry. As the Registered Nurse you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. Primary Responsibilities: Provide high-quality clinical services within scope of practice and infection control standards Coordinate care with other members of the patient/client's care team from admission to discharge Complete clinical nursing assessments per federal/state program requirements and payer needs Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy Develop and revise individualized plans of care/service plans with other community providers Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
