Maintains JACHO standards on the unit Cares for medical equipment and supplies and monitors sterilization of appropriate equipment. Ensures patient safety Escorts patients from waiting area to and from treatment areas Rounds on patients routinely for to assist with falls prevention Aids patients with toileting (including escorting patients to and from bathroom to prevent patient falls) Assists with patient transfers (i.e. wheelchair to/from bed/stretcher; stretcher to/from bed) and bed positioning Provides nutritional snacks for patients as appropriate Collaborates with Medical Director to identify and meet learning needs 7.1 Attends educational programs to advance knowledge and skill 7.2 Adheres to the standards of required training Patient Services Coordinator II: Principal Duties and Responsibilities - Administrative Cross trained to provide back up for all check in and checkout functions as outlined by the MGH/MGPO Front Desk Standards and Cadence Front Desk Check In User without Registration Training Track. Brief summary of purpose: The Assessment Nurse Case Manager completes face-to-face home visits for new enrollees within 30 days of enrollment to onboard and completes regulatory assessments. The Assessment Nurse Case Manager completes in person Health Risk Assessments (HRAs) in accordance with members assigned frequency. The Assessment Nurse Case Manager completes all new Personal Care Attendant (PCA) Assessments using the integrated time for task tool and well as yearly PCA reevaluations. Job Responsibilities Providing excellent nursing care to post-acute residents in areas nearby Supporting residents with basic activities of daily living (ADLs) like eating, personal hygiene, and grooming Protecting resident health and safety by supporting a clean, sanitary, and hazard-free environment Assisting with the transportation and movement of residents around the facility, as needed Documenting and reporting resident vital signs and updating the nursing team on the physical and mental conditions of residents Being accountable to follow and adhere to care plans for residents and all applicable policies, rules, and regulations Engaging with residents in ways that build relationships based on a sense of security and trust Communicating between, and among, staff, residents, and family members in ways that support transparency and the comfort of residents Qualifications and Skills Current Massachusetts Certified Nursing Assistant (CNA) certification Demonstrated knowledge of nursing care procedures and clinical terminology Detail-oriented and able to effectively communicate with medical professionals, residents, and family members Ability to lift a minimum of 50 lbs Ready For a Job That Finally Works For You?
