ESSENTIAL SKILL/KNOWLEDGE FUNCTIONS Assists patients in and out of bed and into wheelchair with the correct use of the transfer belt. Assists patients in bathing; to include bed baths, tub baths, and showers. Assists patients in dressing or undressing. Assists patients in using bathroom and/or bedpan and peri-care. Takes vital signs of patients, e.g., temperature, blood pressure, and respiration and records information. Assists patients to dining area for meals. May be required to assist in feeding of patients and documenting intake of nourishment. Strips bedding and replaces with clean linens. Assists in personal grooming of patients, e.g., brushing hair, trimming fingernails and toenails, and shaving. Turns patient intermittently of patient is bedridden due to illness. Required to clean bedpans and empty catheter bags. Documents and/or reports information regarding the patients’ health, I&O, any change of medical condition, and care given. Distributes fresh water to patients and picks up used water pitchers. Required to pass food trays to individual patients in accordance with diet cards on trays. Provides oral care and denture care. Provides skin care. Assists with admissions, discharges, and transfers as requested. Understands and utilizes care plans. Identifies the four basic food groups on a sample breakfast, lunch and dinner meal. Provides assistance with meals as requested (tray delivery, feeding, etc.). Provides after meal care (remove tray, brush teeth, wash face and hands, etc.). Applies & releases restraints and provides exercise. Provides external/indwelling catheter care. Offers fluids at appropriate times (including routine ice and water rounds). Provides for bowel and bladder training/retraining. Provides colostomy care. Practices proper body mechanics while moving/transferring patients. Provides range of motion exercises for patients. Collects stool and urine samples. Recognizes and reports signs/symptoms of abuse and/or change in condition. Properly documents in accordance with established guidelines. Receives and gives reports on patient’s status or condition. Assists patients to and from activities as requested. Assists patients with walking (with or without assistive devices). Provides care for the dying patient. Provides post mortem care as requested. OTHER CONSIDERATIONS AND REQUIREMENTS In this position, the associate is required to be on his or her feet continually throughout the day. In a full-time nonrestrictive basis, the associate must be able to lift at least 40 pounds of weight, which is required when transferring a patient from a bed to a wheelchair. A transfer belt is necessary and required when transferring any patient. This is for the safety of not only the patient, but also the aide. Other nurses are available to assist with heavier patients as needed. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. 3. Establishes and maintains a therapeutic relationship with the patient and family. 4. Uses evidence-based practices to increase understanding of patient's perceptions of care. 5. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Professional Development 1. Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. 2. Advances clinical competence in nursing practice to progress from novice to expert. 3. Contributes to the profession of nursing through participation in professional organizations. Qualifications Graduation from an accredited Nursing program. Bachelor’s Degree in Nursing. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
