The Home Hospice Intake & Referral Nurse Liaison (RN) meets with patients and families to facilitate the home health admission process. Essential Job Responsibilities Completes an initial assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates referral for patient/family hospice needs. Initiates the plan of care as needed. Educates the patient and family (caregiver) in ways to achieve palliative goals. Provides health care instructions to the patient, family (caregiver) as appropriate per assessment and/or plan. Provides direct patient care as needed. Documents the initial patient and family assessments and interventions. Identifies bereavement and discharge planning needs as part of the care plan development. Prepares clinical notes and updates the primary physician when necessary. Essential Job Responsibilities Completes an initial assessment of patient and family to determine home health care needs. Provides a complete physical assessment and history of current and previous illness(es). Educates the patient and family (caregiver) in ways to achieve home health goals. Provides health care instructions to the patient, family (caregiver) as appropriate per assessment and/or plan. Provides direct patient care as needed. Documents the initial patient and family assessments and interventions. Performs initial communication with the physician regarding the patient’s needs. Ensures that arrangements for equipment and supplies are made. Customer/Referral Source Management: Primarily works with designated hospital and other potential referral sources to develop and promote awareness of home health care services, discuss the agency’s capabilities, and educate on the services provided by home health. Facilitates communication between the referral source and the home health department. Works closely with clinical staff to ensure that necessary services are provided. Job Summary: The BNATP Instructor is responsible for delivering comprehensive training and instruction to students enrolled in our Basic Nurse Assistant Training Program. The instructor will ensure that students gain the knowledge, skills, and competencies required to pass the state certification exam and excel in their roles as CNAs. This position involves teaching theoretical and practical components of the CNA curriculum, and providing guidance and support to students. Key Responsibilities: Develop and implement lesson plans that align with state regulations and the program's curriculum. Deliver engaging and informative lectures on various topics related to nursing and patient care. Provide hands-on training in clinical skills such as patient hygiene, vital signs monitoring, mobility assistance, and other essential CNA duties. Assess students’ performance through quizzes, exams, practical demonstrations, and assignments. Offer constructive feedback and support to help students improve and succeed. Maintain accurate records of students’ attendance, grades, and progress. Ensure that all training equipment and supplies are properly maintained and available for use. Stay updated on the latest developments in nursing education and healthcare practices. Assist in the development and revision of the CNA program curriculum as needed. Comply with all state and federal regulations and guidelines related to CNA training and certification. Qualifications: Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license. Minimum of 2 years of experience working as a CNA or in a related nursing role. Train the Trainer Course Completion Prior teaching or training experience is preferred. Strong communication and interpersonal skills. Ability to engage and motivate students. Proficiency in using technology and educational software for teaching purposes. Excellent organizational and time-management skills. Compassionate, patient, and dedicated to student success. Working Conditions: The BNATP Instructor will work in a classroom and clinical setting. May require evening and weekend hours to accommodate student schedules. Physical requirements include standing for extended periods, demonstrating nursing procedures, and assisting students with hands-on activities.
