Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient Knowledge of high-tech nursing skills as applicable which is not limited to indwelling/straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy. Working knowledge of all infection control procedures and safety precautions. Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Pediatric care experience in the acute and/or home care environment Valid driver’s license Current certification in Basic Life Support for Health Care Professionals.
