The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting. Primary Responsibilities: Develops, implements, and updates patient plans of care in collaboration with physicians and the interdisciplinary team; ensures home‑health eligibility and completes comprehensive nursing assessments (including OASIS and medication reconciliation). Provides and coordinates high‑quality clinical care; supervises and assigns tasks to home health aides and LPN/LVNs in compliance with state and federal regulations, including required supervisory visits. Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care. Participates in orientation and precepting of new staff; contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations. Demonstrates professionalism through punctuality, positive attitude, ongoing learning, meeting deadlines, active meeting participation, adherence to agency standards, and participation in on‑call/weekend rotations. Essential Duties and Responsibilities: Patient Flow Duties: • Review daily schedule and verify availability of supplies and working equipment for the day. • Prepare patient chart prior to appointment time. • Direct patient flow and room turnover in assigned areas. • Communicate with the patients and families and keep them informed of schedule delays. • Maintain cleanliness, inventory and preparedness of assigned exam rooms. • Provide assistance to patients that need help with ambulation or those dependent on a wheelchair or walker. • Communicate changes to patient’s schedule with the provider and team members. • Coordinate interpreter services as needed. • Communicate with families and patients to insure they understand flow and expectations for the visit. • Provide interpretation as requested. • Scheduling appointment as ordered by provider. Direct Patient Care: • Perform vital sign, diagnostic tests and laboratory tests including waived lab test, venipuncture and specimen processing. • Accurately document medical information in patients’ records (flow sheet, progress notes, immunization records, growth charts, refill logs, etc.) lab logs and quality control logs. • Determine immunization status and administer immunization as needed. • Administer medication as ordered by providers. • Label prepackaged medication as directed by providers. • Determine type of appointment needed and prepare patient appropriately. • Assist providers with procedures. • Dispense information to patients as instructed by providers. • Call/fax refill request to the pharmacy as ordered by providers. In-Direct Patient Care: • Check sharps containers daily, replacing when full. • Stock information pamphlets and forms in the clinic daily. • Stock exam rooms, nurse’s station, vaccine administration station and lab daily. • Organize and maintain clean and soiled utility rooms and storage areas. • Disinfect and sterilize equipment. • Maintain proper quality monitoring/control program for sterilization system. • Assist the front desk as needed to expedite check in of patients. • Clean and disinfect exam tables, counters, and equipment such as scales, toys and blood pressure cuff per VGMHC Infection Control Guidelines. • Assemble equipment and prepare trays for minor procedures. • Schedule appointments as ordered by provider. • Track patients’ records as directed by provider. Job Summary: Provides assistance to the surgical team during the intra-operative phase of surgical care for the Northwest Surgical Services region. Works in collaboration with the surgeon and health care team in the performance of surgical procedures, including assessment, planning, directing, providing and evaluating advanced nursing care. All RNFAs will participate in a regional on-call rotation to provide surgical assistance for elective, emergent and urgent surgical cases. Essential Responsibilities: Representative Functions or Duties: Practice standards of performance: Pre-operative. Performs pre-operative assessment and evaluation. Collaborates with the surgical team on plan of care. Intra-operative: Provides knowledge based technical assistance to the surgeon during the operative procedure according to AORN standards, licensure, state regulation and facility/practice protocols, including: Using instruments and medical devices. Providing surgical exposure. Handling and/or cutting tissue. Providing hemostatis, clamp, cut, and ligate tissue as directed by the surgeon. Suturing as directed by the surgeon. Post-Operative. Evaluates the patient post-operatively according to facility protocols. Assessment Standard of Performance & Planning: Communicates information so that continuity of care is maintained. Communicates with patient, family and other staff in a professional and courteous manner.
