ESSENTIAL DUTIES AND RESPONSIBILITIES Assisting the MDS Coordinator Ensuring resident care plan is being followed by interdisciplinary team and monitoring their progress to ensure compliance with MDS process Completing medical forms, charts, and reports in an accurate and timely manner Understanding long-term care reimbursement process including PDPM, Managed Care and Medicaid, triple check, etc. Participation in scheduling of resident care conferences Participating in the QAPI process Monitoring Quality Care Indicators Carrying out quality improvement initiatives Abiding with all facility policies and procedures including not disclosing user ID codes and passwords Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately Participating in facility surveys (inspections) when required and assisting with plan of corrections as well as follow up. Job Description Job Summary: The Patient Navigator-Licensed Vocational Nurse (LVN)-Cancer Clinic works under the supervision of a Registered Nurse (RN), Medical Doctor (MD) or Nurse Practitioner (NP) to provide developmentally appropriate and age-specific direct and indirect patient care in a fast-paced outpatient cancer clinic and infusion center serving patients eighteen years of age or older with suspected or confirmed malignancies or hematologic disorder(s). Performs designated procedures and treatments which require knowledge and technical expertise within the LVN scope of practice and in accordance with established policies, procedures, protocols, techniques, and guidelines. Recognizes and reports signs and symptoms of complications and/or allergic reactions immediately to the RN, MD or NP. Overview JOB SUMMARY Provides the provision of appropriate nursing support and implementation of evidence-based measures that ensure the well-being of patients receiving care and treatment in the interventional procedural areas and within radiology imaging, which includes adequate and safe monitoring of patients before, during, and after interventional procedures. Interventional Radiology Nurses (IRRN) need to have an understanding of nursing process, providing the standard of care, and guaranteeing quality improvement and exercises influence over departmental activities. MINIMUM QUALIFICATIONS Education: See licenses and certification Experience: Six months experience in critical care or emergency room (within the past three years) preferred. Interventional Radiology nursing experience preferred. Knowledge and Skills: Able to communicate effectively in English both orally and in writing. Able to work and multitask in a fast-pace environment; effectively managing, prioritizing and delegating patient care and completing work assignments and other duties, as assigned. Has a general knowledge of all areas of nursing practice to include understanding of sedation. Demonstrates knowledge of common disease process, treatment modalities and interventions. License/Certifications: Current active California Registered Nurse License, current American Heart Association (AHA) BLS and ACLS. EKG course, and PALS certification is required within 90 days of hire/transfer date. Primary Duties and Responsibilities The Clinical Nurse II - Critical Care role provides direct patient care in accordance with the Nurse Practice Act for assigned patients in a critical care unit including Neuro ICU, CCU, and Sub ICU. The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care. This includes providing physical assessments, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned. Utilizes the nursing process based on evidence-based best practices to meet patient care needs. Evaluates and adjusts the nursing care process as necessary with consultation as needed, and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions. Maintains a working knowledge of departmental and hospital standards, policies and procedures. Performs other duties as assigned. In addition to the above responsibilities, the Clinical Nurse II is involved in Hoag Committee/Council with at least 50% attendance, communicates quarterly to UPC (Unit Practice Council) on committee/council work, and completes three supplemental activities a year.
