Advanced Practice Provider (APP) - (Transplant Outpatient)

Position Summary The Advanced Practice Registered Nurse – Certified Nurse Practitioner (APRN- CNP) or Certified Physician Assistant (PA-C) reflects the mission, vision, and values of University of Illinois Hospital & Health Sciences System, adheres to the UI Health Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. The ideal candidate holds a degree from an accredited Nurse Practitioner or Physician Assistant school or college and has Board Certification from Nationally Accredited Certifying body, based on the APP category. The APP functions as part of the multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced care to patients. The APP is responsible for the health outcome of an assigned patient population and consults and confers with supervising physicians in an area of clinical expertise within the APP's scope of licensure. The APP is required to have and maintain a faculty appointment in one of the schools or colleges of the University of Illinois in order to receive clinical privileges to practice at the University of Illinois Health System. The scope of care is general family medicine, which includes care for all ages and including but not limited to, routine preventative care and management of chronic disease management. Duties & Responsibilities Scope of Practice: Works within the Bylaws of the University of Illinois Hospital & Health Sciences System and in adherence to hospital and divisional policy. Works collaboratively with other providers in the care of patients. Performs an accurate history and physical examination of patients, comprehensive or episodic health assessments and develops patient care plans. Performs procedures according to clinical delineation of privileges. Provides prevention strategies and management of acute and chronic health problems. Orders and interprets tests, treatments, and therapies within the scope of practice. Prescribes medications, diet, supplies, prosthetic devices, and other home health aides or medication according to clinical privileges. May provide other clinical duties within the scope of practice as directed under the emergency operation plan. Demonstrates fiscal responsibility to positively impact the financial performance of the organization while ensuring optimal patient care and safety. Professional: Integrates evidence-based knowledge and clinical expertise in decision-making. Participates in peer review and competency assessment activities. Establishes an effective relationship with the entire care team. May lead and assist in research and evidence-based practice through participant selection, coordination of the protocol(s), data collection, and other associated activities. Demonstrates accountability for professional conduct. Advances professionalism by seeking opportunities to develop new skills and to enhance current ones. Teaching: Provides comprehensive teaching strategies for patients and their families. May provide education and mentoring of undergraduate and graduate students and current and new employees, including but not limited to medical staff, residents, advanced practice providers and nursing. Provides formal and/or informal education or mentorship to both internal and external groups pertinent to area of specialty. Communication & Leadership: Participates in program development. Acts as a resource for clinical staff in the practice setting. Consults with management in assessment, problem-solving, decision-making, and evaluating clinical/system issues and models of care. Identifies and participates in quality-improvement initiatives within the practice setting and initiates change as appropriate. Leads multidisciplinary teams to provide integrated delivery of patient care services. A staff nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. The staff nurse assumes the primary responsibility and accountability for all patient care personally provided and that provided by those directly under her/his supervision. Works directly with the Wound Care Department to identify the care needs for patients throughout the continuum of care Directs flow of the outpatient Wound Care Department Develops and maintains competence to provide direct care to the wound care department patient population Provides direct care if needed and assigns appropriate staff to participate with this care Directly mentors / educates staff and monitors competence on an ongoing basis Acts as a liaison between the wound care staff and the wound care provider/residents and other departments to develop and ensure a smooth process for the safe and effective care of patients in the Wound Care Department. Manages the phone calls to the department and documents on returned calls to patients Manages DME orders for patients receiving care in the Wound Care Department Manages benefit verification for advanced modalities; ensures patients have complete and correct documentation prior to BV submission Acts as additional chain-of-command resource of the Wound Care Center covering select duties for the Clinical Nurse Manager during his/her absence, working effectively with the nursing and other departmental leadership Alternate to charge reconciliation and tissue bank tracking Acts as a resource to nursing staff in problem solving resolution of patient care issues Provides some supervision and evaluation of clinical skills of nursing staff In addition to coordinate the care of wound care patients: Collaborates with admitting, the Bed Coordinator, and/or the Supervisor to facilitate the plan for timely placement of admissions from the wound care center Ensures adequate and appropriate coverage for the unit that considers census, acuity and RN patient ratio. Plans for and makes unit assignments based on staffing, competency of staff, coverage of following shifts Delegates assignments to appropriate RN based on criteria and adjusts patient care assignments as necessary Delegates work to non-professional staff (tech, Unit Secretary) Counsels professional and non-professional staff and gives feedback when necessary Coordinates staff and their duties during direct patient care - routine and emergency What you will need: Education: Graduate of an approved school of nursing. Bachelor / Master of Nursing-prepared preferred Certification: Current license as a Registered Nurse in Illinois. American Heart Association BLS Experience: Minimum of two years experience as a Wound Care Staff Nurse Unique or Preferred Skills: Demonstrates a commitment to the mission of Swedish Hospital and demonstrates a positive service orientation and adheres to all responsibilities and standards of the Hospital.
