SIGN-ON BONUS INCLUDED POSITION OVERVIEW: The Jewish Board strives to provide person-centered care that considers clients’ physical and mental well-being. The Family Nurse Practitioner’s role will be to evaluate clients in our Youth and Families Division for medical conditions and provide treatment according to established best practices; impart education around healthy living and disease prevention guided by a trauma informed approach that also incorporates harm reduction and motivational interviewing; and collaborate with mental health clinicians to effect the best health outcomes for the populations we serve. Clinical duties will be in the Supervised Independent Living (SILP) for young adults currently in foster care in the Bronx, and two group residences: Kaplan House in Manhattan, and Cedar Knolls House. KEY ESSENTIAL FUNCTIONS: Work scheduled hours at two locations: Kaplan House and Cedar Knolls House. SILP youth will be seen in one of these group residences. Most work will be in-person, but some sessions may be provided via telehealth. Provide comprehensive primary care services, including detailed histories, physical exams and diagnoses. Incorporate emphasis on preventative care and medical screenings based on established guidance. Order and interpret diagnostic tests, using results to formulate treatment. Treat acute and chronic conditions, with appropriately scheduled follow-up and monitoring. Make referrals to subspecialist care when indicated, then incorporate subspecialist recommendations into comprehensive approach. Prescribe medication in accordance with best practices. Consider range of treatment modalities and interventions that could potentially improve clients’ conditions. Educate clients about their conditions, as well as therapeutic and preventative measures they can use to improve their physical health. Utilize motivational interviewing techniques such as SBIRT in addressing behavioral change. Remain up-to-date with innovations in primary care and nursing practice. Understand the interplay between mental health conditions and somatic health, especially the physical side effects often caused by commonly prescribed psychiatric medications. Communicate as needed with multiple disciplines including case managers, social workers, psychiatric providers and medical subspecialists in a way that ensures a holistic understanding of the clients’ condition(s). Maintain medical records promptly and accurately, utilizing the organization’s Electronic Health Record. Rotating responsibilities for addressing urgent issues after-hours Initial assessment at admission and annually thereafter (more often when clinically indicated) Provide medical health assessment (including differential diagnoses, prognosis), treatment recommendations and treatment planning for residents. Provide health education and counseling about health and sexual/reproductive health Participate in practice improvement projects as initiated by the program and/or the agency Medical Standards and Quality Improvement departments. Monitor client progress; recommend discharge and aftercare planning medical needs. Communicate needs of youth with program staff, and interface with 29i clinic director.
