Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. About Dr. Joseph Germano Dr. Joseph Germano serves as Regional Director of Electrophysiology for Suffolk County and specializes in the diagnosis and treatment of complex heart rhythm disorders. He works closely with St. Francis Hospital & Heart Center leadership to expand access to advanced electrophysiology services across Long Island, with an emphasis on early and definitive treatment strategies. Dr. Germano is board certified in Clinical Cardiac Electrophysiology and Cardiovascular Disease . Responsibilities Perform comprehensive patient assessments, histories, and physical examinations. Order and review diagnostic testing, including ECGs and cardiac monitoring. Collaborate with electrophysiology cardiologists to develop and manage treatment plans. Provide pre- and post-procedure care for patients undergoing EP procedures, including ablations and pacemaker/ICD implantation. Manage follow-up care for patients with arrhythmias and implanted cardiac devices. Educate patients and families regarding treatment, medications, procedures, and ongoing care. Maintain accurate and timely clinical documentation. Collaborate with physicians, APPs, nurses, and other members of the healthcare team. Qualifications Current New York State Nurse Practitioner license and registration. National NP board certification. DEA registration or eligibility. BLS and ACLS certification through the American Heart Association. Strong clinical, communication, and organizational skills. Cardiology or electrophysiology experience preferred but not required. Ability to work at both Smithtown and Westbury locations.

Nurse Case Manager

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Develop and update individualized nursing care plans and contribute to person-centered service planning (ISP). Educate staff on health-related issues, proper medication administration, infection control, and other medical protocols. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician.
