Is responsible for adherence to all State and Federal Guidelines for Skilled Nursing Facilities. KEY RESPONSIBILITIES INCLUDE: Direct Resident Care: Is ultimately responsible for the assessment of resident’s needs (MDS) and development of resident’s plan of care with goals to ensure the residents are able to achieve optimal care and services. Certified Nurse Assistant (CNA)

As directed by the team leader and the Registered Nurse and in partnership with the health care team and patient/ family, performs work processes to accomplish positive patient outcomes, and organizational goals. Provision of care focuses on psychosocial support and activities of daily living. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. We are currently hiring for both shifts, Full and Part Time. Maintains basic technician skills, such as obtaining vital signs. Reports observations, including any change in residents condition to Registered Nurse in a timely and accurate manner. • Performs interventions to enhance psychosocial support. Assists residents to Activities. • Provides/assists with care to support physical function for activities of daily living, such as toileting, bathing, feeding, and ambulation which allow residents to achieve their highest practical level of function. • Documents according to facility guidelines to accurately reflect care provided immediately after performing procedure or task.
