Demonstrate understanding of and utilize proper infection control practices/policies. Accompany residents, as needed, to various activities and functions. Complete required documentation accurately and in a timely manner. Maintain a clean, safe, and secure environment for residents. Act in compliance with all corporate, state, federal, and other regulatory standards. Demonstrate respect and compassion in every interaction. Duties and Responsibilities Act in the capacity as a Team Leader in a modified primary care setting. Provide regular resident status updates to appropriate personnel. Supervise your team and participate in team member evaluations. Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment. Assist in the implementation of an individualized treatment plan for each assigned resident. Comply with evaluation, treatment, and documentation guidelines and complete required documentation in an accurate and timely manner. Attend and participate in department/facility meetings, as required. Act in compliance with regulatory and professional standards and guidelines. Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency. Adhere to facility policies and procedures and participates in facility quality improvement and safety programs. Oversee Falls Program and Pressure Sore Program for those residents on their team. Demonstrate compassion in every interaction. Can initiate and/or assist in CPR. Can initiate fire alarm and function in the disaster plan as instructed. Maintain all infection control measures in providing patient care utilizing Universal Precautions. Operate equipment required for implementing patient care consistent with operating instructions and safety regulations. Perform as a team leader, including: a. assign patient care duties to personnel on the health care team based on patient needs b. receive, record and transcribe orders from the physician c.. receive, prepare and issue a change of shift report d. assist with orientation of newly hired LPNs and nursing assistants. Promote self-growth and increased professional knowledge through attendance at in-services, workshops and seminars. Respect and maintain patient's dignity, individuality, privacy and confidentiality. Be knowledgeable of Ohio House Bills 600 & 822, resident's rights. Promote positive public relations for the facility. Maintain conscientious punctuality and attendance within the standards of the personnel policies.
