Performs follow-up phone calls to patients after appointments as indicated. Works with participating sites to develop mechanisms for the sites to share the patient data (for those requiring follow-up) in a manner that utilizes their existing data and optimizes their time and effort. Completes all necessary electronic records and paperwork regarding patient interactions and appointments. The Hospice CNA is responsible for observing patients, reporting these observations and documenting observations and care performed.
