The Personal Care Aide is responsible for providing patient assistance with nutritional and environmental support and personal hygiene, feeding and dressing and/or, as an extension of a self-directed patient, selected health related tasks in his/her home or facility under the supervision of a registered nurse. Responsibilities: Carries out the Agency's mission, philosophy, goals and objectives within guidelines of Agency policy and position function. Interprets and implements the Agency's philosophy to staff and members of the community. Participates in the development and implementation of a realistic goal-oriented plan of care. Provides personal care and rehabilitative services and assists in activities of daily living as directed by the nurse or therapist. Follows care plan instructions as directed by the nurse. Provides Environmental Support: Make/ change bed linen, occupied or unoccupied. Light housekeeping: Patient areas to include kitchen, bath, bedroom, etc.; dust, mop, sweep, vacuum, clean kitchen sink, counter, stove top, table, refrigerator, clean bathroom sink, tub, shower, toilet. Clean up after meal preparation and provision of care. Personal laundry: clothing and bed linen, wash, iron, and mend, prepare for laundry service. Maintain safe environment and clean patient equipment. Provides Nutritional Support: Develops menu. Prepares grocery list, shop. Prepares meals/ snacks. Assists with feeding. Measures and records intake of food and fluids. Personal Care Functions- assists with or performs and records: Skin care. Grooming: hair shampoo, beard, nails, shave, oral hygiene, denture care; assist with dressing, apply elastic stocking, clean glasses. Toileting: bedpan, urinal, commode or toilet, adult diapers. Wound dressing: change clean dressing (band-aid, gauze pad and tape); Assist nurse with changing sterile dressing Performs and records simple measures: Blood pressure on arm Vital signs: temperature, pulse, respiration Collect specimens: stool, urine, sputum (without suctioning) Measure intake and output Test for urine sugar and acetone Weigh patient Carries out maintenance exercise program. Assists with use of medical supplies and equipment; assist nurse with changing sterile dressing. Walking: assist with our without assistive equipment, walk inside or outside of home, use of safety belt. Provides Related Supportive Tasks: Accompanies out of doors and to medical appointments. Assists with socialization and recreation activities. Assists with completion of personal business activities such as telephoning, correspondence, paying bills. Observes, records, and reports patients condition. Other Activities: Participates in selected case conferences. Maintains daily patient record and timesheet. Promptly informs nurse of changes, needs or problems with patients condition and/ or environment. Performs related activities as directed by the nurse. Adhere to care plan instructions. Qualifications: Written evidence of having successfully completed a New York State Department of Health approved: Basic Training Program in home health aide services; Or, Equivalency Examination or Competency Only Program in home health aide services; Or, Written evidence of having successfully completed a New York State Department of Social Services approved Training or Competency Program in personal care services; Compliance with Inservice Education requirements; Minimum of 18 years of age.

Essential Functions Develop and oversee system-wide competency frameworks that ensure consistent clinical practice standards in the facility. Develop, implement, and evaluate education programs that support hospital operations, patient throughput, and clinical excellence and aligned with organizational goals. Coordinate orientation, ongoing competency development, and annual training programs for staff. Collaborates with nursing leadership, nursing staff and other disciplines to organize Nursing Education, Quality Improvement and Research programs. Lead the design and implementation of competency assessment tools, skills validation, and performance improvement initiatives to enhance patient care and quality outcomes. Ensure compliance with federal, state, and accreditation requirements related to staff training and hospital operations. Exhibits expertise on practice and regulatory compliance: Advocates for top of license practice. Nursing Practice Creates and implements a shared vision for Nursing Professional Development throughout the facility. Advances the nursing profession by increasing nurse-led evidence-based practice projects and research studies throughout the hospital and supports dissemination of the work through publications and conference presentations at the regional and national level. Supports the leadership in developing units practice council, Shared governance model to engage and empower front line staff in development of NDNQI data metric collection, and quality improvement plans. Creates and develops evidence-based policies, procedures, protocols, and best practice models/guidelines using advanced clinical and specialty knowledge. Leads and supports in designations efforts of the institutions such as Pathways to Excellence, ANCC and or Magnet Designation Participates in Nursing Quality Improvement activities. Consultant In conjunction with senior leadership, establishes role accountabilities and expectations, performance standards, and methods for measuring/monitoring of standards for nursing in collaboration with Nursing leaders and CNO Collaborate with Nursing leaders, Infection prevention, QI Directors, physicians, and other healthcare professionals to identify educational needs and provide educational support to staff. Responses to regulatory mandates for research and management of sentinel events Responds to identified needs of Carewell Health to promote quality patient care and safety. Facilitator . Serve as an instructor and facilitator for organization-wide certification and continuing education programs (e.g., life support, emergency response, and other regulatory or specialty certifications) as required by role, department, or accrediting bodies. Support clinical educators and preceptors across the system in delivering high-quality competency-based education. Collaborates with interdisciplinary leadership team members to develop and execute initiatives addressing the learning needs of professional nurses to meet the needs of the diverse patient population across the region. Change Agent Utilize innovative training methods to support diverse learners. Leader Leads committees, work groups and/or projects related to the implementation of educational activities. Leads operations for the academic interface for schools and colleges of nursing in planning for clinical rotations. Evaluates program/student placement effectiveness on an ongoing basis. Working Hours Monday to Friday 8 am to 4:00pm Must be willing to differential hours work holidays, weekends and oversee the overnight shift to meet the needs of the unit. Minimum Education/Certifications/ Work Experience Current RN license (multi-state compact or state of practice). Certification in Nursing Professional Development (NPD-BC), NE-BC, NEA-BC or equivalent is a desired or preferred certification. 7+ years in healthcare, with at least 5 years in nursing or hospital leadership. Strong presentation, communication, and facilitation skills. Demonstrated ability to influence change across clinical and operational teams. Demonstrated success in developing and delivering staff education programs. Strong knowledge of regulatory/accreditation standards (e.g., CMS, Joint Commission). BLS, ACLS, and PALS instructor preferred. Physical Requirements Physically able to sit, stand, stoop, and walk for long periods of time. The team member will need to frequently assist lifting patients and equipment when working on the unit. Must be able to work in a stressful environment, work independently, and be capable of critical thinking, making sound decisions, detail oriented, alert and self-motivated. Ability to push/pull up to 50lbs, carry and lift upto 50lbs.
