Overview The Certified Nursing Assistant assists the clinical staff in the care of and treatment for patients in an acute care setting under the supervision of a licensed nurse. Responsibilities may vary based on department assignment. Responsibilities Patient Care Activities Participates in nursing reports by receiving and contributing to status report of patients. Communicates abnormal findings or changes in patient status in a timely manner to supervising RN. Prepares patient and/or patient room for the admission, transfer or discharge process. Provides morning and evening care as well as general care routines such as bathing, personal hygiene, etc. in accordance with established nursing care procedures to assure patients' physical comfort. Takes and records vital signs, pulse oximetery, pain level, height, weight (and other measurements, as applicable) in an accurate and precise manner. Prepares patients for meals, assists in feeding patients, and distributes water and other nourishment according to patients' needs. Performs general care activities. Consistently follows hand hygiene and isolation practice guidelines established at WHMC. Makes rounds, anticipates patients' needs and responds to call lights promptly. Assumes responsibility for patients' clothing and personal property (prosthetic devices, hearing aides, glasses, etc.), securing items in accordance with. Ambulates patients as per plan of care. Assists with or transports patients as necessary. Collects appropriate specimens and labels specimens at the bedside properly. Transports in a proper and timely manner. Reports and/or records any changes observed in condition or behavior of the patient. Utilizes restraints as per physician order and only as a last resort. Appropriately monitors and assures appropriate device is used. Frequently toilets patients. Ambulates patients. Utilizes lift equipment appropriately and safely for all lifts/transport. Applies and utilized gait belt appropriately to assist patients. Ensures accurate and timely documentation using the electronic medical record and assuring use of own login at all times. Initiative and Dependability Independently performs assigned duties, assists staff in deliver of more complex treatments and procedures. Demonstrates initiative by recognizing and performing duties which need to be performed, although not directly assigned. Seeks guidance and direction, as necessary, for performance of duties. Utilizes Onstage/Backstage techniques for communication with patient, family and hospital staff. Consistently strives to understand, anticipate and meet the needs, expectations and satisfaction levels of patients and other customers. Quality (Performance Improvement, Risk & Patient Safety) Is aware of and participates in the performance improvement activities of the department. Completes incident reports regarding complaints or other issues. Actively promotes a culture of safety by speaking up when safety concerns are. Participates in departments "Survey Ready" efforts. Professional Conduct, Growth and Development Completes Mandatory Requirements (Such as Annual Updates, Employee Health Screenings, license/certification updates and other requirements within. Job Summary: The ICD Nurse Specialist role at Kaiser Permanente Southern California Region is an advanced practice nursing role with the critical element being expert clinical practice. The role is built around the five role subcomponents as regulated by the California Board of Registered Nursing: Clinical Practice, Research, Education, Leadership, and Consultation. The ICD Nurse Specialist functions under the medical direction of the Electrophysiologist and/or Cardiologist evaluating, educating, and performing device programming, for patients with implantable cardiac devices (ICD and pacemaker). Essential Responsibilities: To ensure that nursing care needs of the cardiac device patient population are met, provides 24 hour clinical leadership by developing standards of practice, policies and procedures for cardiac device patient population that are consistent with professional practice standards. Implementing methods and processes in specialty area to support change in nursing practice, care programs and clinical innovation. Achieving improved patient/family outcomes. Independently assess cardiac device transmissions, including electrograms, from patients using knowledge and experience in device programming. Functions independently in the assessment and interpretation of cardiac device data from patients in outpatient clinic, inpatient hospital, and by remote interrogation. Provides medical and/or technical programming recommendations and advice. Performs interrogations and reprogramming as needed, on all types of implantable cardiac devices. Evaluates clinical practice in cardiac device patients to ensure community/professional standards are met. Effectively identifies, evaluates and addresses disease prevention and health promotion issue of the cardiac population with implanted devices, while administering quality patient care. Schedules, coordinates, and performs interrogation. Interprets, and evaluates therapeutic and diagnostic tests on cardiac device patients working with the Electrophysiologists and/or Cardiologists. Explains necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. Records physical findings and formulates plan, based on patients condition. Consults and collaborates with physician and other healthcare providers as needed regarding patient care management issues and in the development of comprehensive patient care plan. Submits healthcare plans and goals for periodic review by physicians. Maintains accurate and confidential medical records. Documents all procedures, evaluations, treatments, outcomes, education, and referrals, consistent with NCQA (National Committee for Quality Assurance), JCAHO (Joint Commission on Accreditation of HealthCare Organizations) and state regulatory standards. Analyzes new knowledge gained from conferences, workshops, professional literature, or hands on training and assimilates this knowledge into clinical practice. EDUCATION & EXPERIENCE Associate Degree in Nursing from an accredited school of nursing required Current, unrestricted LVN license in California required Intravenous (IV) Certification required Basic Life Support (BLS) required Minimum of one-year post-graduate LVN experience required Recent infusion experience preferred QUALIFICATIONS & SKILLS Keen assessment and monitoring skills; ability to identify medication reactions quickly. Skilled at performing intravenous catheter placement and monitoring for signs and symptoms IV site related complications. Skilled in drawing labs using aseptic technique. Skilled at performing subcutaneous injections and monitor for site reactions. Able to demonstrate infection control precautions and sterile technique. Able to provide age-appropriate education to pediatric, adolescent, adult, and elderly patients. Demonstrate strong communication skills in speaking and writing. Effective interpersonal and organizational skills with the ability to work independently as well as part of a group Strong computer skills, including word processing, efficient use of the internet and email, and experience with Electronic Medical Records (EMR) ESSENTIAL FUNCTIONS Greet patients respectfully and ensure patients are cared for in a timely and safe manner. Provide general nursing care to patients; administer prescribed medications per drug protocol and within the scope of practice Perform an age vital signs and nursing assessment on each assigned patient. Provide age specific education; initiate and reinforce as needed for infusions/injections they are receiving. Review list of biologic questions for all patients. Provide patient education about individualized treatment and answer questions appropriately according to evidence-based practice and the Medication Policy and Procedures. Provide written education materials about the drug they are receiving. Follow the Five Rights prior to medication administration. Observe, record, and report patient's condition and reactions to drugs and treatments to the Medical Director and record in electronic medical record (EMR). Complete all other required documentation and reporting as necessary. Provide accurate, real-time documentation in the patient's EMR Provide close monitoring of patients in infusion rooms and prioritize complexity of care. Maintain timely flow of patient care and documentation. Respond to emergencies and confidently demonstrate and document interventions ordered by the Nurse Practitioner and/or Provider. Follow all OSHA guidelines to minimize exposure to blood borne pathogens. Follow HIPPA regulations and practices with a high level of integrity and honesty in maintaining patient confidentiality. Assist and support colleagues with all aspects of patient care participate and complete other duties as assigned The hourly pay range for this role is expected to be between $30 - $38. Actual base pay could vary based on factors including but not limited to experience, subject matter expertise, geographic location where work will be performed, and the applicant's skill set.
