This position is a 100% onsite opportunity . You will need to be based in the greater Chicagoland area. A staff nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. Patient centered care principles and professional nursing practice standards provide the foundation for the nurse to assess, plan, implement and evaluate the care provided. The staff nurse assumes the primary responsibility and accountability for all patient care personally provided and that provided by those directly under her/his supervision. Essential Job Functions Utilizes appropriate assessment skills to determine patient needs. Develops a plan of care for those patients whose needs do not require the expertise of a Case Manager or collaborates with the Case Manager to develop a plan of care for specified patients. Provides input into the development of clinical paths with assigned Case Manager. Uses care plan and/or clinical path in providing patient care. Delegates as appropriate based on patient needs and competencies of staff. Competently performs patient care according to hospital policy and nursing practice within clinical specialty, as documented by the respective clinical skills inventory evaluation and completion of competency days. Evaluates care given and adjusts plan of care to meet the changing needs of the patient. Documents appropriately in patient care records. Prepares and administers medication/treatment according to the physician's orders, utilizing professional knowledge base to assure patient safety. Works collaboratively with other patient care and multidisciplinary team members to provide patient centered care. Documents or ensures documentation of all of patient care from admission to discharge according to hospital policies. Incorporates discharge planning into clinical pathway or plan of care to provide patients with smooth transition from hospital to home. Provides cost-effective care by appropriate utilization of time, supplies and personnel. Evaluates patient care interventions and collaborates with all team members to make changes as necessary. Initiates and/or participates in patient care conferences. Assumes the responsibility to interact on a consistent basis with the patient and family providing coordination of the plan of care for those patients whose needs do not require the expertise of a Case Manager. As an Advanced Practice Provider , you will be responsible for comprehensive direct patient care management. In this role, the APP will manage a caseload of patients and exercise autonomous decision making in the assessment, diagnosis and initiation of delegated medical treatments. The scope of practice includes daily management as primary team, discharge planning, patient education, staff education, cardiac surgery consultation and research. The APP will utilize advance practice skills obtained through the completion of a formal provider program. These skills include advanced levels of assessment, planning, intervention, and evaluation of care. The APP is also responsible for planning, organizing, and directing patient care efforts in collaboration with medical and surgical physicians and other team members. Sign on bonus up to $10,000.00 and relocation available Essential Job Functions Provide daily management of patients undergoing Cardiac, Vascular and Thoracic procedures, including heart and lung transplantation and ventricular assist devices. Gathers pertinent information systematically and skillfully from all sources: history, diagnostic tests, comprehensive physical examination, and psycho-social assessments. Differentiates normal and abnormal variations for all body systems. Cardiac, Vascular and Thoracic procedures, including heart and lung transplantation and ventricular assist devices. Assesses patient and family adaptation, coping skills and the need for crisis or other intervention. Accurately interprets diagnostic tests. Analyzes multiple sources of data in making clinical judgments and in determining the effectiveness of a plan of care. Develops problem lists with associated differential diagnosis. Identifies educational needs of the patient and family and assists with teaching. Incorporates assessment findings into a plan to direct patient care. Formulates a plan of care in collaboration with the patient, family, physician, nursing staff and other member of the health care team which incorporates health care maintenance, discharge and follow-up care. Plans and implements appropriate pharmacological therapies. Accurately and appropriately performs routine diagnostic and therapeutic techniques according to established protocols and standardized procedures for nurse practitioners. Initiates referrals based on patient and family need. Seeks appropriate consultation dependent upon patient health care needs. Accurately presents and documents the database, impression and plan of care. Promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of adolescent, adult, and geriatric patients. Participates in research projects in the practice area. Perform other related duties and initiatives as deemed necessary in the best interests of patient care. We are seeking a collaborative and dynamic Clinical Nurse Educator – Diabetes with current CDCES credentialing to support our inpatient diabetes education program across adult and pediatric populations. Reporting through the Clinical Nutrition and Diabetes Patient Education leadership team, this role serves as an educator, consultant, mentor, advocate, and clinical resource focused on advancing diabetes care, improving patient outcomes, and supporting professional nursing development. In partnership with nursing leadership and interdisciplinary teams, the Clinical Nurse Educator – Diabetes provides specialized education in diabetes management, including insulin administration, glucose monitoring technologies, and insulin pump therapy. The role also supports the ongoing development of nursing and clinical staff through evidence-based education, coaching, and mentorship to promote safe, high-quality patient care.
