Communicate with physicians, patients, families, and the care team to ensure coordinated care. Help prevent avoidable hospitalizations and emergency room visits through proactive patient care. Deliver compassionate, high-quality care that supports positive patient outcomes. What is Required? Current State License as a Licensed Vocational / Practical Nurse Graduate of an accredited Certificate, Diploma, or Associate School of Nursing 1 year of experience in a clinical care setting Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Experience in a hospice or home health environment is preferred What's in it for you?
