About This Position The Medical Assistant will manage the patient flow throughout the clinic and assist providers in the management and treatment of their patients. Patients come from a variety of backgrounds and various age groups requiring individualized care including infants, children, adolescents, adults, or geriatrics. The focus of the Medical Assistant is on collaborative, supportive and delegated tasks as described in the Blaze Health Medical Assistant scope of practice. Delegable tasks and competencies are specific to the role of the Supervising Nurse and Supervising Provider and are described by the Minnesota Board of Nursing and the Minnesota Board of Medical Practice. The Medical Assistant receives direction and collaborates with the Clinic Supervisor, Clinic Manager, and Providers to meet the defined goals of the customer, shift, department and organization. Communication with the health care team related to progress, concerns, questions or problems that arise are an essential element of the Medical Assistant role. Unit based skills list outlines specific expectations including technical procedures with competency assessment/validation. Our rehab department consists of over 150 physical therapists, occupational therapists , and speech-language pathologists, who work collaboratively with a pediatric physiatrist and physiatry nurse practitioner. Rehabilitation Services are provided at both our Minneapolis and St Paul Hospitals as well as six outpatient therapy clinics. Children’s Pediatric Rehabilitation Medicine program brings together outpatient support for children with complex needs and dedicated inpatient consultative care to support the acute needs of hospitalized patients. Ambulatory clinics at Children’s MN include primary care clinics that are the longest running Health Care Home in Minnesota for children with complex chronic health care needs; spina bifida clinic; NICU follow up clinics; and post-fetal (in utero) clinical care. Conducts eligibility assessments against protocol inclusion/exclusion criteria, coordinating required screening tests (e.g., echocardiography, cardiac CT, catheterization, laboratory studies) with clinical departments. Obtains informed consent, ensuring patients and families understand the trial, randomization (where applicable), study procedures, and alternatives, and documents the consent process per regulatory requirements. Follows enrolled patients through the life of the trial, from screening and index procedure through all protocol-required follow-up visits (e.g., 30-day, 6-month, annual), coordinating visit windows, imaging, and assessments to maximize retention and protocol compliance. Monitors participants for adverse events and clinical changes, communicating findings promptly to the PI and treating clinicians. Ensures protocol requirements are met, including direct contact with research participants, collection of health information, coordination of study-related procedures, source documentation, and data entry. Prepares and submits reportable event reports (e.g., adverse events, serious adverse events, protocol deviations) to sponsors, IRBs, and regulatory agencies in accordance with the study protocol and established policies, procedures, and regulations. Manages day-to-day conduct and oversight of multiple concurrent structural heart trials, demonstrating strong organizational skills, independence, and resourcefulness. Collaborates with sponsor clinical research associates during monitoring visits by providing study documentation, data clarifications, and reports as necessary. Coordinates handling of study devices, administration of clinical trials, collection of source information, and case report form completion, including timely data entry into sponsor EDC systems. Job Overview Opened in 2018, Amira Choice of Plymouth is one of our newest Ebenezer senior living communities. We are located at the intersection of Old Rockford Road and Highway 55 in the Northwest suburb of Plymouth, MN. We offer 94 units of Independent, Assisted Living as well as Memory Care and Care Suites. Managed by Ebenezer, Minnesota's largest senior living operator This Nursing RN schedule includes; casual / on call minimum of 3 shifts per month 1 holiday per year Responsibilities Maintains current assessment and documentation of resident health status: Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements. Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement. Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually. Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST. Maintains clinical charts and progress notes using computerized documentation system. Communicates changes in resident conditions to medical and other relevant individuals and perform the appropriate follow up to health needs. Coordinates resident health services: Facilitates coordination of services with internal and external providers. Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations. Promote skill building for medication self-management. Provide cross-training to ACT team on topics related to the integration of mental health/ physical health and medication. Work with clients on all aspects of psychiatric rehabilitation, support, and psychotherapeutic interventions identified in the treatment plan. File reports with Vulnerable Adult, the Ombudsman’s Office, Child Protection, and other mandated/required reports when appropriate.
