This position is salaried with bonus plan and additional visit rates. As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations. Primary Responsibilities: Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team Receives referrals, ensures appropriate clinician assignments, evaluate patient orders, and plot start of care visits Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Annual base starting salary range: 100k-155k Responsibilities Perform patient evaluations, this includes but not limited to - Good faith exams, medical history, patient desired results, treatment plans, and explanation of aesthetic results, risks, and benefits. Perform aesthetic services such as dermal fillers, neurotoxins, micro-needling, and medical-grade chemical peels. Maintain accurate medical documentation, such as patient charting, before & after photos, and treatment records. Prioritize patient safety above all else: inspect the injection room and act to maintain excellent hygiene and safety. (decontaminating equipment, sanitizing surfaces, preparing injection room etc.) Honor and respect the diversity of patients and their individual rights to care Acknowledge and remedy mistakes and seek to maintain and advance your skills and understanding within the industry. Prepare injectables and treatment trays as needed. Assist with maintaining inventory levels as needed. Must be available Fridays and Saturdays; open availability Skills Proven experience as a Nurse Practitioner Knowledge of aesthetic nursing care methods and procedures Knowledge of health and safety guidelines and procedures Kind and professional responsible and compassionate Strong organizational and multitasking skills Patience and excellent problem-solving skills Valid NP license Must have an up to date Medical Assistant Certificate in the state of Florida Must be willing to travel to various facilities in the state of Florida Must have Nursing Home experience for at least 1 year (Please do not apply if you do not have this) Must be personable, as this is a concierge, client facing job. As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends. Eligible for full time benefits The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
