Hospice RN Case Manager Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager . As a vital member of our hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities. This position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation. MAIN FUNCTION: Fulfills patient care responsibilities as assigned, including organizing patient flow, preparing exam rooms, accompanying patients to exam rooms.
