Care Coordination & Navigation: Serve as the primary point of contact for patients and families, and, additionally, act as a liaison with the multidisciplinary care team (physicians, advanced practice providers, nurses, social workers, child life specialists, psychologists, pharmacists, financial counselors, research coordinators, transplant coordinators).
