A L&D Scrub Nurse is a Registered Professional Nurse (RN) who manages patient centered care that is outcome directed. Patient centered care principles and professional nursing practice standards provide the foundation for the nurse to assess, plan, implement and evaluate the care provided. The staff nurse assumes the primary responsibility and accountability for all patient care personally provided and that provided by those directly under her/his supervision. Essential Job Functions: Performs scrub functions, which involves preparing, selecting, and handling instruments and supplies used during surgical procedures, and conducting surgical counts with the circulating nurse. Carries out circulating functions before surgery, including preparing the operating environment, orienting and assessing the patient, reviewing the patient's medical record, consent, and surgeon's notes to ensure all prerequisites and preferences are met. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Under the direction of the Interventional procedure providers and in collaboration with the program support team, coordinates the care of patients through their interventional procedure. Acts as a clinical liaison for patients going through this process in the clinic and in the in-basket messages. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Evaluates and manages patient pre and post op care. Essential Job Functions Provides care coordination & patient education to patients in heart and vascular service line. Facilitates resolution to patient care gaps and monitors follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients. Acts as liaison for patients, families and their multidisciplinary clinical team. Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan. Develops and/or coordinates development of patient education materials. Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an emergency room visit. Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients in cardiology. Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.
