Ensures that all patients who are able, are sitting up and are comfortable for the meal. Picks up meal trays in a timely manner. Feeds patients who are unable to feed themselves. Provides ice water at least once every shift to bedside for patients who are able to have ice water. Removes additional fluids from patients on fluid restrictions. Offers/Provides bath, linen change to patients who require assistance or complete care, and documents on activity record. Notifies licensed personnel if patient refuses bath and/or linen change, and documents on activity record. Obtain intake and output as directed by physician or plan of care. Update patient board in room daily. Assists in admission process by obtaining vital signs, providing personal care items and room orientation. Utilization of critical thinking skills in prioritization of care. Ability to identify changes in patient condition and notifies licensed personnel of any change in patient condition. Provide safe environment. Documents all patient and family teaching as requested/necessary. Demonstrates correct use and troubleshooting of equipment. Participates in the development, prioritization, and evaluation of individualized plan of care. Assist in modification of plan based on patient's response. Provides information directly to the physician and documents appropriately. Ability to take directions from licensed and administrative professionals. Attempts to resolve conflict independently, if unable, seeks the assistance of department leader. Provides health care at the best value without compromising patient care. Utilizes the Pyxis appropriately, obtains items under specific patient's name and returns unused items. Responsible and accountable for all actions. Obtains vital signs, ascertains patient complaint or reason for visit and enters the information into the patient’s record. Checks the patient record to insure all required information has been obtained and completed. 2. Completes disability status sheet and fax to proper places, as requested. 3. Writes school/work excuses and provider orders, and has provider sign. 4. Prior to patient visit, performs the following: a. Ensures that all equipment in exam room is clean and properly set up. b. Ensures supplies are adequate. 5. Cleans and restocks examination rooms for the day’s use. Reviews patient records, x-rays, and test results for the day’s visit and prepares necessary paperwork. 6. Accompanies the provider while seeing patients, when appropriate. 7. Screens telephone calls for the provider, and takes clinical messages, as appropriate. 8. Answers patient inquiries with respect to medical questions within the scope of practice and practice policy. 9. Takes calls from the pharmacist. Calls or electronically submits prescriptions and prescription refills to pharmacy as directed by the provider. 10. Coordinates diagnostic and laboratory services for patients on an outpatient basis such as PT, MRIs, Bone Scans, CTs, etc. and completes proper paperwork. 11. Assists provider in examination and surgical or diagnostic procedures and treatment, as well as preps patients. 12. Marks superbill or documents EHR for any procedures and injections performed and any supplies used. 13. Makes arrangements with proper agencies for administration of patient home health care as directed by provider. 14. Coordinates provider referrals to other providers and relays proper information about the patient. 15. Maintains all clinical equipment to include sterilizing, as applicable. Evaluates all equipment for damage or maintenance needs, and reports information to the Practice Manager. 16. Checks clinical supplies and initiates the ordering process. 17. Via electronic, paper or other forms of receipt reviews and completes clinical mail for the provider when appropriate/authorized. 18. Prepares patient insurance forms completely prior to giving to provider for review and signature. Provides patient care in an office setting under the supervision of the medical provider. Meets the communication needs of the patient / family and medical provider. Maintains and follows regulatory requirement and policies and procedures. Communicates with medical providers and staff about patient’s clinical condition.

Reviews and implements daily nursing needs as outlined by resident Care Plan or nurse managers. Transports residents or assists residents with mobility needs. Turns and repositions bedfast residents, alone or with assistance. Takes and records temp, B/P. Pulse, resp rate., Ht., Wt., and food and fluid intake and output, as directed. Monitor residents’ activities and reports any irregularities of resident performance to nurse managers. Participates in and receives nursing report upon reporting and departing to/from work. Reports all changes in residents’ conditions to supervisor Cleans all equipment used and returns to proper area after use. Maintains cleanliness of residents’ units, central bathing area, treatment and supply rooms, workrooms, and linen closets. Provided post-mortem care as instructed. Accurately records all care as directed. Participates and attends periodic in-services and staff meetings. Understands and adheres to the guidelines of “Residents Rights”. Perform additional duties as assigned. Education/Requirements: Prior experience in LTC preferred but not required. Able to effectively communicate with residents and nursing staff.
