Registered Nurse - Admissions Discharge - Full Time

Reviews all orders on new admissions, transfers, and discharges. Communicates patient changes and condition with the assigned team members as indicated Communicates with patient/family within 24 to 72 hours of admission. Attends daily interdisciplinary team rounds. Collaborates with Primary RN, case managers/social services during the patient's stay, Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population and facilitates access to outpatient resources that the patient may need, in collaboration with the multidisciplinary team.
