Job Description The Heart Failure Navigator is a specialized clinical professional responsible for coordinating and optimizing care for patients with heart failure across the continuum—from inpatient care through discharge and outpatient management. The role focuses on improving clinical outcomes, reducing readmissions, achieving LOS targets, and ensuring adherence to evidence-based heart failure guidelines. The Interventional Psychiatry RN reports indirectly to the Outpatient Behavioral Health Clinical Manager and may be assigned to assist in the Outpatient Behavioral Health Clinic. Meets all mandatory training and education within defined time frames, including departmental staff meetings. 4. Demonstrated ability to safely prepare and maintain IV lines, administer medications and/or other interventions. 6. Ensures all daily tasks and activities are completed during work shifts. 7. Participates actively in creating a therapeutic and harmonious environment by demonstrating teamwork and cooperative, responsive behaviors. 9.
