Job Description The CHHA Department is currently seeking a full time CHHA Registered Nurse to join their team! Eligible newly hired UFT nurses will receive a $6,000 hiring bonus, distributed in two phases: the first installment of $3,000 will be provided after successfully completing six months of employment, followed by a second $3,000 payment upon reaching one year of service. Job Summary: Responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care. Duties And Responsibilities Provides and coordinates cost-efficient patient care that is and reimbursable under the patients’ insurance plan and approved by a physician. Verifies insurance plan information, crosschecks patient Insurance ID card at home. Accepts case assignment depending on clients’ needs regardless of geographic location. Case assignment is all inclusive of CHHA (Certified Home Health Agency) requirements, including initial assessments, revisits and documentation on each day of service.
