Description A nursing student, who has completed the first clinical year, inclusive of medical – surgical nursing, within a baccalaureate nursing program and has a fundamental knowledge of the art and science of nursing, can function in this role. This experience assists the student nurse in expanding previously learned theory and skills through supervised patient care within a work setting. Responsibilities 1. Performs and records patient’s vital signs, such as blood pressure, pulse, temperature, respirations, pulse oximetry and pain rating 2. Documents clinical data as assigned, in a computer or manual system 3. Performs clinical procedures, such as specimen collection, venipuncture, EKG, application of sequential compression sleeves, post-mortem care, falls prevention, maintenance of skin integrity and capillary blood glucose monitoring 4. Sets up for selected procedures and assists RNs accordingly. 5. Assists patient with activities of daily living, such as feeding, bathing, and/or toileting. 6. Orients patients to Specialty Service/Unit environment. Assists patient with ambulation and all levels of mobilization 7. Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian 8. Maintains accurate and timely systems for transmission of patient dietary orders and changes. 9. Assists in pre- and post-operative procedures, such as patient transport, completion of pre-op checklist and reception of patient from PACU/OR. 10. Utilizes measures to insure patient safety and promotes a restraint free environment 11. Maintains use of restraints for patients under the direction of a RN and according to set policies and procedures 12. Assists in emergency resuscitative procedures as directed by the registered professional nurse 13. Provides care on the deceased as well as the involved family/significant other 14. Performs pertinent observations/specific treatments of procedures, as assigned. 15. Documents patient care delivered on appropriate flow sheets 16. Assists the professional registered nurse in the collaborative plan for patient education participating in patient/teaching activities 17. Maintains a safe, clean and functional environment, including ensuring for the comfortable movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary 18. Performs One-to-One/Constant Observation and Safety Observations of patients as ordered, e.g., for safety, elopement, or suicide prevention 19. Performs correct infection control techniques and maintains isolation status for patients under direction of a RN and according to set procedures 20. Operates the pneumatic tube system when required 21. Overview The Registered Professional Nurse is responsible and accountable for prescribing, delegating and coordinating all nursing care as defined by The New York State Nursing Practice Act. Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. POSITION OVERVIEW We are looking for full-time Case Managers to work within a domestic violence residential & legal program. This program focuses of providing legal and job training programs for domestic violence victims, residential and vocational programs for individuals with developmental disabilities. The Case Manager is responsible for direct service provision to residents. Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Baccalaureate Degree in Nursing is required. Masters in Nursing or health related field is preferred. Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Overview Under the supervision of the Nurse Manager, Critical care Nurses provide direct and individualized nursing care to patients based on the application of scientific nursing principles. Management patient post neuro procedure, or Neuro patient transferring from Surgical intensive care unit. Have knowledge of all vasoactive Drugs such as Vasopressors, Inotropes, Catecholamine, and Adrenergic. Calculate and administer according to the guideline. Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays, such as intracranial pressures, arterial line pressures, central venous pressures, pulmonary artery pressures, Balloon Pump, Hypo/Hyperthermia machine, CVVHD, Peritoneal Dialysis. Initiates patient education plan, as prescribed by physician. Teach patients and significant others how to manage their illness/injury, by explaining: post-treatment home care needs, diet/nutrition/exercise programs, self-administration of medication and rehabilitation. Demonstrates efforts in the prevention of hospital acquired conditions (HAC) CAUTI, CLABSI, Pressure injuries and Falls. Records all patients’ medical information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms. Prepares equipment and assists physician during examination and treatment/procedures of patient at the bedside. Participates in the formulation, review and or revision of departmental policies. Proactively participates in performance improvement initiatives to continuously improve the quality of patient care. Uses hospital resources effectively in providing patient care. Follows the process of reporting broken equipment. Recovering patient after sedation analgesia. Transferring patient to floor after verbal report/electronic report using Hand-off. Involves Patient and family in discharge teaching. Discharge patients safely if out patients. Performs other duties as assigned within the RN scope of practice. Qualifications Education: Internal & 1199 SEIU: Graduate of accredited nursing program(s). Bachelor's degree in Nursing preferred. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient's condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. 9. Maintains patient/employee confidentiality in the management of information. 10. Develops the plan of care based on the diagnoses and individual patient needs: 11. Records, reports, and interprets the clients' responses to the plan of care. 12. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. 13. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. 13. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT 1. Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. 2. Demonstrates accountability for own nursing practice. 3. Considers ethical issues of professional nursing practice and adheres to the code of ethics. 4. Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. 5. Serves as a resource person to nurses and other health team members. 6. Participates in peer review. 7. Incorporates research findings in clinical practice. 8. Participates in approved nursing and medical research studies. 9. Keeps knowledge current by self-directed learning, literature review and attendance at educational programs. 10. Participates in defining, maintaining, and interpreting standards of nursing practice. 11. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. 12. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. 13. Seeks opportunities to enhance professional nursing practice and influences outcomes. 14. Treats co-workers, patients and families with dignity and respect. Qualifications At least two years of nursing experience in area of specialty practice applied for. Nurse Practitioner experience of at least one year preferred Licensure: New York State License with current registration as a registered nurse. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient's condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate. Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), The Neonatal Resuscitation Program (NRP) certificates as required by specialty.
